How to taper of Sertraline?

Posted by Hordak on December 3, 2018, at 16:33:18

Hi,

so I am at the beginning of the process of tapering off that stuff. Question: What is the best way?

Skaehill and Welch, 1997, suggests the following:

"Reduce by 25 mg every two weeks until dose is 25 mg/day, then 12.5 mg every two weeks"

What are your suggestions?

I've been thinking that the following would be more advisable: Reduce by 25 mg every two weeks until dose is 50 mg/day, then 12.5 mg every two weeks.

Oh right, there is a german version, too. They hate meds but they know alot about withdrawing!

I remember that one. I once posted a thread there, asking for the best antidepressant, way before taking any antidepressants myself. Reaction was quite rude ^^ *hahahaha*

Yeah that sounds like them. I cant really pick a side in this fight... I do think that most psychmeds are very unpredictable to say the least. However they do sometimes help. But they can also inflict damage.

Since we are all different, I do not like going by a pre-arranged schedule that somebody else thought was a good idea. Instead I like for each patient to determine their own speed at weaning based on their difficulty. For some it's fairly easy. For others it's extremely hard.

For me it was hard. Zyprexa was way harder. But no matter what the drug, my best approach is to lower the dose in tiny steps over an extended period of time. Lowering the dose by 25mg is way too big a giant step. I would do it more like 6mg. Or less.

You can smash the pill into powder and divide the powder into roughly equal piles. They don't need to be exact. 4 piles equals about 6mg each (assuming a 25mg tab). 5 piles equals 5mg, 6 piles equals 4mg.

I like to reduce dose slowly in tiny steps - way tinier than the docs do it - because they've never actually experienced the hell they make their patients experience. And if at any time I feel confident I can do it in bigger steps. I can modify my plan as I go. If I run into withdrawal troubles, I can stabilize at my current dose or go back up to my last previous dose and wait a few days before continuing to wean.

I like the 10% less every 4-6 weeks method. or even 7.5%

True... The question is does it need to be tapered quickly i.e. to make way for a new drug? If not, then why rush?

You can really mess yourself up by withdrawing too quickly.
