SLS and other resistant : opiods or stimulants

Posted by zonked on September 28, 2016, at 5:16:06

Hi all dr. Excuse the voice recognition I'm too tired to type on this phone. The first question is for SLS... Scott have you ever been on potent opioids for pain? I am beginning to wonder if you would be a candidate for Subutex or Suboxone treatment. At times I've entertained testing positive for heroin so that I can get on a medicine that has helped many people escape depression. Similarly I was recently cycled from nordelta parnate I was recently cycled from nordelta parnate however 60 milligrams does not do it I need a hundred and 20 milligrams and I am so pissed at being depressed that I've been taking that. I'm going to be very Frank with my next office visit and say hi either need to be prescribed Vyvanse which will take care of my ADHD and depression long as you carefully of course as Focalin XR did not work this time particularly at 5 milligrams. Or I will discontinue my current insurance plan and pay a fortune for a PPO to see a private psychiatrist because I'm tired of suffering I'm tired of hospitalizations I'm tired of hating myself I'm tired of Life tasting like paper period thoughts anyone?

Hi Zonked.

I am very, very sorry to hear that you have been having such a difficult time. It sounds like you have been in a lot of pain and frustration.

I have not tried any opioids. I asked my doctor for buprenorphine (Subutex). He said not.

What country do you live in? Is there any possibility that you can take intranasal ketamine? According to what I read, a large percentage of people who were refractory to previous treatments respond robustly to ketamine. Others glean a partial improvement. I was in the minority who don't respond at all to it. A friend of mine who is in remission using intranasal ketamine had been a partial responder to a combination of Wellbutrin, Lamictal, and Abilify. Have you ever tried this combination?

One of the things that occurs to me is that you might be bipolar or have an occult bipolar diathesis. Very frequently, ADHD/ADD and bipolar disorder occur comorbidly. Even if you are not bipolar, using lithium as an augmenter to Parnate might help. Combining Parnate and lithium has been used successfully for decades. In the case of unipolar depression, low dosages of lithium (300-600 mg/day) can be sufficient. I find that 300 mg/day is perfect for me. If I go any higher, I experience dysphoria, emotional numbing, and apathy. I don't respond at all to Parnate unless I combine it with either desipramine or nortriptyline.
