Risperidone tiredness - split the dose?

I'm on 1mg of risperidone to augment paroxetine for panic disorder and I must admit it knocks me out quite a bit. I find it hard to keep awake in the afternoons which is difficult when you're looking after a 3 year old! I currently take the whole 1mg at night, would perhaps splitting the dose half in morning and half at night make a difference to the tiredness?

Splitting the dose would lead to a lower blood level of risperidone in the early morning, so would perhaps be more useful if you were finding it difficult to wake up. I suspect it would not make too much difference to afternoon tiredness. Even so, you have nothing to lose by trying it.

Assuming that risperidone is helping, it might be worth trying a lower dose in order to reduce the sedation eg. 0.5mg to 0.75mg per day. Of course, you may already have tried this.

Why take an antipsychotic for panic disorder-most of the studies show mixed results. Why not augment with something like Gabitril or Lyrica. Do you take benzos?

I should have mentioned that I also take paroxetine and clonazepam. It is possible that the clonazepam is causing the tiredness. I only take a small dosage to augment. I've been through all the SSRIs (they all poop out after about 3 years) and the doses of clonazepam that I've had to take when they do have been so high that they've knocked me out. I tried valproate - but that actually made things a lot worse. I'm not 100% sure, but I think both the drugs you mentioned aren't subsidised where I live (NZ) hence they weren't an option.

I'd say cut the dose down to 0.5mg for a week or two, then go back up to 1mg, and be prepared for drowsiness for a week or so after the dose increase

The drowsiness is because risperidone has antihistamine properties, this drowsiness should pass off after a couple of weeks as the body gets used to the drug
