Geodon (ziprasidone) - What are your experiences?

I am curious to know what are people's experiences with Geodon (ziprasidone). It seems to be a rather unpredictable drug.

1. Diagnosis?
2. Dosage?
3. Therapeutic effects?
4. Side-effects?
5. Reasons for discontinuation?
6. Would you recommend it to others?

Bipolar Depression.

40 - 80 mg/day

> 3. Therapeutic effects?

Mild to moderate antidepressant effect during first week followed by relapse.

> 4. Side-effects?

Cognitive blunting, brain-fog, derealization

> 5. Reasons for discontinuation?

Lack of efficacy, cognitive disturbances.

> 6. Would you recommend it to others?

It would not be my recommendation as a first-line treatment for any condition. I believe that it has a place in the treatment of schizophrenia where negative symptoms predominate.

No.

I just thought it might be valuable to the Psycho-Babble community to collect some anecdotes as information to help make treatment decisions.

I am usually disappointed whenever I try to do this.

1. Diagnosis? ADHD, Bipolar 1, OCD
2. Dosage? 40mg, 80 mg, 160mg
3. Therapeutic effects? anxiolysis, anti-manic, mood uplift.
4. Side-effects? mania
5. Reasons for discontinuation? did not treat OCD.
6. Would you recommend it to others? yes

Hi Scott, When I first went on Geodon, it was like a miracle. Almost overnight, just 20mg h.s. Stopped my depression in it's tracks, wiped out my anxiety and virtually made me well. A couple of weeks later it wasn't working quite so well and we had to raise it to 40 mg. This went on, bumping it up by 20mg every few weeks till I reached my maximum tolerable dosage of 120 mg akasthisia above that). I kept it at 100 mg for a long time and am now trying raising it again-I had dropped down to 40 mg for a few months in hopes it would start working again later if I raised it back y up. It's only been one day but I'll let you know how it goes. I sure miss that great window back in 2008-it was almost like a mild, controlled hypomania with no downside.
Dx; mdd, bipolar II, GAD, panic disorder, treatment resistant depression, atypical depression. I definitely would recommend it. No side effects at all. Oh, except a prolonged qt interval on EKG but felt nothing bad.

At the time, bipolar nos most recent episode mixed.

80mg. Started as 40mg am and 40mg pm but it was so sedating we switched it to 20mg am and 60mg pm. The higher the dose the more sedating it was for me.

Calmed me down, and worked fairly quickly, I would say felt better by the third day of treatment.

The extreme sedation was the worst side effect. I know it does something to the heart (qt interval) but not sure how that may have affected me.

I needed something that was going to help the depression end of things, and Geodon did nothing really to help depression.

Well based on my experience I'd say if you are having an acute manic or mixed episode it could be useful. I also had no weight gain, I took it for about 3 months. Not sure about its use as a long term med. Not an antidepressant for me.

I think that this sort of thread represents one of the most potent uses of this forum. I'm glad you thought it valuable enough to offer your comments. In the past, I have been disappointed by a lack of participation.

diagn: varies, bipolar/mixed, agitated dep, MDD, psychotic depression

dosage: 80mg and 160. (i think that was it..been several years)

at 80 did nothing to help depressive symptoms
at 160 i had psychotic symptoms, severel obsessed on committing suicide (key word obsessed), bad energy, visual illusions frequent.

only psychosis as side-effect, maybe some constipation.

discontinued due to psychosis.

i would recommend it but with caution to know symptoms as it snuck up on me then hit hard.
