Vyvanse dosing above 70mg-- sources to support it?

My doc, and even pharmacist when I asked, said Vyvanse can only be prescribed up to 70mg, but that doesn't make any sense-- that's only equal to 10mg Adderall IR 3x/day, and when I took Adderall, I took 20mg 3x/day.

My insurance company did a prior authorization, and although it's not on the formulary (it was for Medicare, which permits off-formulary prior authorizations)-- the insurance company said, when I asked, that the prior authorization is good for up to 180mg / day for the drug.

So is the PDR telling docs 70mg is the maximum to prescribe, and other sources are telling my insurance company 180mg?

Shire is about to do Phase III trials for Vyvanse for schizophrenia, and those trials are going up to 200mg.

I'm about to switch docs for other reasons, so if I could give my new doc an authoratative study or overview of the med's dosing... That would be help.

Only thing I've found on short notice. Probably won't help. I'm on 120mg.

Wouldn't the loads of data on dexedrine apply?

you'd think.... apparently, my insurance's prior authorization (I called) is for up to 180mg, so even my insurance thinks some discretion is due by doctors to go over 70mg.. don't tell that to my doctor, who thinks she has to follow the PDR. Even Shire, I think, doesn't intend docs to do that. They went for tiny doses that pass clinical trials with flying colors.

Shire doesn't say what Dexedrine is equal to, but does say Adderall XR 30mg is equal to Vyvanse 70mg.

I was on 2 70mg in the morning for about 6 months and then switched to dexedrine spansule 4 15mg [60mg] daily spaced out to last 12 hours.

Vyvanse is diffrent in the absorbation of how it takes effect....but its still d-amphetamine just a diffrent delievery system. It usally takes a bit longer to take effect than dexedrine from what I rerember.
