Update

Posted by delna on November 18, 2009, at 12:12:40

Hi guys,
I have good news- I have fever!!! I threw up and have a slight temperature and headache with pain behind my eyes!

Yes, I am weird and am happy because this may well explain why I have been getting more tired than usual since yesterday. At least it means that the increased exhaustion is not just depression getting worse by the minute!

I am seeing someone in the morning who will investigate this (just a passing infection i suppose) but will also tell me what tests to have to rule out chronic infections or deficiencies.

Thanks for all your concerns and suggestions on my threads. I will get back to you ASAP. Am ill so am going to sleep now.

I saw my pdoc today. He said loads of things but in a nutshell, I am starting on prozac with the provigil, then adding reboxetine later and then eventually lowering lamictal slightly- thats the plan for the moment.

I also saw a specialist in movement disorders today (for the Geodon related tardive pain) who 'explained' my medical mystery to me. Not good news but at least now I know- no APs for me at all. (This was confirmed by Dr Ivan Goldsberg who I had written to and who replied to me today.) So no Geodon ever :-( Or Abilify..

The specialist thought my level of sleepiness was absolutely ridiculous so he arranged an appointment with a sleep specialist for Monday.

I hope something treatable comes out of all these investigations.

Thanks for listening and for all your best wishes and concern.
I can't thank you enough. I don't know how I would be surviving this period without you all.
Lots of love
Take care
D

I look forward to hearing the explanation you were given regarding your medical mystery.

One step at a time. I wish you luck with the Prozac. It is an underutilized drug that really does stand alone from the other SSRIs, especially because it produces a sustained elevation of dopamine and norepinephrine in the prefrontal cortex.

> I look forward to hearing the explanation you were given regarding your medical mystery.

Well, he couldn't explain it as such but he did give me his professional advice pretty strongly. He said that even though I had an existing sensation which he says does sound like restless genital syndrome, he feels the conversion to pain looks like tardive pain syndrome, regardless. He used the analogy of RLS and said that it is only rarely aggravated by APs. Furthermore it doesn't *change* form, it just becomes worse. He feels that the *conversion* to pain was a TOTAL change in form and presentation so did not look simply like an exacerbation of the existing movement disorder.
He couldn't explain why the sensation went away in the first place with Geodon or why the pain occurred in the same stop as the sensation was- he said that was a mystery. But he strongly advised me to stay away from all APs except clozapine. I believe that my pdoc in Ny who prescribed the Geodon and knew about the pain also believes it's TD (which he totally missed)since I sent him the paper..In fact he was upping the dose which was essentially masking the TD- the worst thing you can do.He never told me to stop the Geodon- it was a decision I took myself because the pain was unbearable. Furthermore he handed me a prescription for Abilify (which is apparently a big no-no for anyone who has had TD before- according to Dr Ivsn Goldsberg) Otherwise there is no explanation why he is avoiding me like the plague and ignoring my emails pleading for his expert opinion/ guess. Maybe he thinks I am out to get him.
Anyway I drafted a letter to the author of the paper on 'genital tardive pain syndrome' and had my pain specialist send it to him. But now I am pretty convinced that APs are out for me (apart from clozapine)

Thanks
Scott, I'm so sorry to treat you like a pdoc but I don't know what to do. My symptoms are changing daily even though I am on the same drugs. One day I am completely exhausted, the next much more awake with bad anxiety. Now it seems like I am suddenly not tolerant to even 200mg of Provigil- it is giving me palpitations (a few weeks ago I was on 400mg + other stimulating things). My anxiety never presents with palpitations so it's not the anxiety. The same thing happened to me post stopping Geodon- I suddenly couldn't tolerate even 25mg of Provigil whereas I was used to 300-400mg daily. Regardless to say I was a zombie for 6 months till one day I randomly re-challenged myself and found I was tolerant again.
Now I am daily(or every few days) fluctuating wildly.Today I needed to knock myself out with Klonopin whereas a few days ago I was sleeping 20 hours with no dose changes.
I am scared to try the Prozac because I feel every time I try something new my chemistry (which is already so weird and unstable) changes dramatically. So I really don't want to mess with drugs-especially since I am prescribing them to myself. But I am getting worse so I need to understand this.
My diagnosis is BP rapid cycling so i don't know if its just that and if I need another mood stabilizer. Do you understand this strange thing at all. My pdoc can't explain it (nor does he even try to)
Thanks for any thoughts
Sorry again
Take care
