Clomipramine + acne and others problems (Scott)

Posted by Vincent_QC on February 23, 2009, at 9:18:07

Hi Everyone,
Well, I have some questions about the Clomipramine.

I'm now at 50mg since more than 1 1/2 weeks, I feel ok, I see no difference on my anxiety or depression level but I experience strange side-effects.

The first thing I notice is that I have a lot of waves of Epinephrine (adrenaline), especially when I express myself in a social situation and that what I say can hurt someone (being bitchy)... or when I think about a situation that make me feel frustrated or things that I lived in my past that I can't forget and who make me feel a lot frustrated... Is it normal? I have the goosebumps all over my body and I feel pins and needles sensations on my head when all of this happen...It happen also with no apparent reason or in no special situation...when I watch TV or things like that...

I also have a lot of pimples since 4 days, and no, I don't change anything in my food intakes, I eat always the same things...In fact, I gain weight already and I eat less because I have some nausea in the morning...I normally have a pimple or two, especially after I shave my face, but now it's terrible...I have tons of pimples everywhere in my face...Is it normal?

So, I asking someone who know more things about TCA's than myself...especially Scott...

For Scott, do you think I should switch to the Nortriptyline insistead of the Clomipramine, since the Clomipramine hit more the serotonin and in the past everything who just hit the serotonin fails (Cipralex, Celexa, Prozac, Zoloft, Paxil)???

My Family Doctor said that adding some Doxepin can help me because he hit more the NE...or adding some Atomoxetine (Stratterra) will also help because it's a reuptake of norepinephrine...

What do you think about these two siggestions??? Should I just switch to the Nortriptyline because of it's NE effect...or add something who will hit just the NE under the top of the Clomipramine?

Thanks for any advise you can give to me !!! I appreciate it a lot!

Bye!

Vincent ;-)

Hi Vincent.

> I'm now at 50mg since more than 1 1/2 weeks, I feel ok, I see no difference on my anxiety or depression level but I experience strange side-effects.

You may not see any improvements until you reach 125mg.

> The first thing I notice is that I have a lot of waves of Epinephrine (adrenaline), especially when I express myself in a social situation and that what I say can hurt someone (being bitchy)

I don't want to say that this is normal, but I imagine the increased NE available is putting you in a "fight or flight" mode. It should lessen over time. I experienced something similar with idazoxan, a drug similar to Remeron.

> ... or when I think about a situation that make me feel frustrated or things that I lived in my past that I can't forget and who make me feel a lot frustrated... Is it normal?

Not normal, but plausible. Do you find that you are dwelling on arguments or having arguments with people in your head?

> I have the goosebumps all over my body and I feel pins and needles sensations on my head when all of this happen...

Yes. In "fight or flight" mode, this can happen as blood is shunted away from the skin and diverted to the muscles. The hairs stand on end as a response to the sudden cooling of the skin - just like when you get hit with cold water or first come out of a pool.

No normal. You might be sweating more or exuding more sebum and attracting more dirt. Try washing your face more often.

Are you OCD?

I really don't know what you should do right now. It depends on what conditions you are suffering from.

What is your family history of mental illness?

Hi do 2 mistakes in my post.

The first med my Family doctor recommend as an add on to the Clomipramine is the Reboxetine , not avaible in the Canada but he say that it will be easy to obtain as an import med...

The second mistake is that I forgot to mention that I gain 10 pounds in 1 1/2 weeks... I always gain weight on any AD's I try, especially the ones in the SSRI's family, Cipralex (Lexapro) was the worst as well as the Paxil...but well that's a lot of weight...I don't want to ended with another 40-50 pounds + the 50 pounds I gained since the last fall.... I can't live with that side-effect...that's seem that even if I don't eat a lot I gain weight...

So it's why I ask about the Nortriptyline, I know it can cause more sedation, but is it less hard on the weight gain??? For the cardiac effect, I assume it can't be worse than the Parnate experience I had 2 months ago... In fact, with the Clomipramine I don't monitor my blood pressure but I know that it's probably more elevated than usual...and for the sedation I don't care, if I can take the whole dosage of Nortriptyline at bedtime (depend of the half-life of it), it will maybe help me to sleep more well at night...and stop the Seroquel as a sleep aid...

Thanks again for your answer and your help ;-)

Bye!

Vincent ;-)

Well, it's what I think also...but before I reach that dosage, maybe a switch to Nortriptyline will be more helpfull for me since no SSRI's worked on me...

I understand that...but it's strange no, it's happen often and more when I live a stressfull situations or events.

Sometimes...I always feel that the others people look at me and judge me...That's a part of my social anxiety problem...

So you think it's the NE effect? I always think that the Clomipramine was more effective on the SE with a weak or no NE effect...???

> Are you OCD?

No, I just have a Body dismorphic problem...similar to OCD I think... but no real obsessions...or things that I Tend to repeat or do too often...

> I really don't know what you should do right now. It depends on what conditions you are suffering from.

Ok, that's the difficult part... First my main problem is social phobia with a panic disorder problem with agoraphobia and panic attacks, depression appear more after I get addicted to the benzos drugs...and I have also a general anxiety problem with BDD (Body dysmorphic disorder).

For now, my main problem is the BDD and the general anxiety with the usual lack of motivation, the lack of energy at daytime, the insomnia at night and the constant anxiety level, alone or in social events...
