can methylphenidate be combined with SJW?

Is there any interaction between a low dose of SJW (3-600mg) and low dose methyphenidate (5-10mg)?

I can't find any evidence for an interaction. Methylphenidate is not metabolized by the P450 cytochrome enzymes, so the SJW induction of 3A4 can't be of any relevance. I did find one paper, a lone case report of a possible reduction in efficacy of methylphenidate by SJW, but there was no abstract, so I can't tell what it said. The decrease in efficacy may have been due to subjective additive effects, rather than an interaction per se.

I would have no hesitation giving it a try.

Wierd when put into drug checker St John's Wart came out as an MAOI? Any idea why? Phillipa

Fifteen years ago, a lab researcher reported that SJW was an MAOI, but he didn't make clear that he was using concentrations hundreds to thousands of times what you would achieve from typical doses. It is an MAOI, but too weak to be of any clinical significance. Green tea has a stronger MAO effect than SJW. Neither is anything to be concerned about.

Lar so since I drink two bottles of green tea a day and now am more tired and trying to cut down on nighttime benzos and take the 50mg of luvox. Think green tea is safe right? As I've been so puzzled and experimenting with the dose of valium and xanax and tireder each day to sleeping til almost noon. But don't get to sleep till 3am. Concern no???? Thanks Phillipa

There are several compounds in SJW which inhibit monoamine oxidase to varying degrees. Was the study you refer to considering the whole plant extract?

Anyhow, I have only heard that Green Tea inhibits MAO-B. Do you know of any studies suggesting it inhibits MAO-A as well.

Green tea is intersting. There is a 5 fold dopamine boost with green tea. It inhibits MAO-B, COMT and DAT activities. In addition, theanine displays some NMDA antagonist activity. Caffeine also causes dopamine release.

Just DON'T combine green tea with methylphenidate. I'll never make that mistake twice.

Green tea is safe, but the caffeine might be a concern in insomnia. Why cut the benzo if you're not sleeping well?

Yes, it was. I couldn't find the questionable study, but I found the one that dismissed the earlier work. No one has ever replicated what the first study claimed.

No, as far as I know, the effect is limited to MAO-B. And very mild.

I think the biggest impact arises through COMT inhibition, but I'm sure the combination is potentiated.

Lar problem all of a sudden sleeping too well and always tired so thought some stimulation from green tea and cutting down on benzos. Not a good idea? I just found this about green tea too. Phillipa

I see no reason at all to not try it, if that is what you are inclined to do. Ritalin might offset some of the lethargic effects of SJW, if you got those.

As far as I understand, the MAOI effects from SJW are only found in the crude plant, but not in extracts. I believe it is mostly in the stems. Only cheap brands that mix in whole plant powder might have some of that. Minute amounts might be found in extracts. Regardless, the effect is insignificant. No one ever got MAOI type reactions or side effects from SJW that I am aware of.

The best use of SJW I ever got was when it was added to 20mg prozac and 5mg zyprexa. Needless to say, there were some significant drug/herb reactions there, which caught up to me about 6 weeks later with what felt like a tripling of the Zyprexa dose with many of the high dose antipsychotic side effects, not to mention a close-call with serotonin syndrome.

But hey, you're talking about combining SJW with Ritalin, which in comparison to what I did is child's play, so I wouldn't have any concerns beyond the usual normal cautions of taking any med or herb.

Lots of things inhibit MAO to some extent. In almost all cases it is irrelevant and can be ignored.
