Please read my attachment on zoloft/lamictal??

Posted by ricker on October 26, 2008, at 22:31:19

I am currently taking 100mg zoloft, 100mg lamictal, 15mg remeron and 0.5/bid clonazepam.
The following arcticle suggests zoloft can double the blood level of lamictal?
Does this mean I'm actually getting a 200mg. dose of lamictal?
Thanks for all replies..... Rick

Department of Psychiatry, UMDNJ-RWJMS, New Brunswick, NJ 08901, USA.

Blood level monitoring helps to determine the therapeutic and toxic ranges for anticonvulsants and antidepressants. We investigated initial drug-drug interactions between lamotrigine and sertraline. We report on case histories of two epileptic patients who were initially on lamotrigine and to whom sertraline was added to control psychiatric features. In case 1, a total daily dose of 25 mg sertraline, with nondetectable sertraline and desmethylsertraline blood levels, resulted in a doubling of the lamotrigine blood level with symptoms of toxicity. In case 2, a 25 mg reduction in the total daily dose of sertraline resulted in halving of the lamotrigine blood level even though the lamotrigine dosage was increased by 33%. This shows that sertraline has potent interactions with lamotrigine metabolism. The authors hypothesize that inhibition of glucuronidation is responsible. Clinicians are advised to observe for symptoms of toxicity and to do serial blood levels to monitor this interaction.

Ricker did drug checker with zoloft and lamictal and only one potential interaction came up and that was grapefruit juice so I think you're okay but best to check with your doc or pharmacist. Love Phillipa

I would take this with you the next time you go to your pdoc and discuss this.

I could impact any future decisions regarding the use of these meds together.

Seldom.

Thanks for your replies. I will take the literature to my pdoc although I'm sure his reply will be as usuall, "STAY OFF THE INTERNET"??

I have given lamictal a fair trial and believe I will be going off it. Acne outbreaks, my face begins to glow a nice red and feel slightly hot at 150 mg. much like a slight summer time sunburn.
The top of my back/base of neck feels like it's in a vice, chronic tightness for weeks.

My appointment is in 2 weeks so I am going to taper off the lamictal. At my last appointment, he made mention of trying an antipsychotic to augment the zoloft for depression??

I'm not psychotic......yet! ):

Rick

Ricker same thing mine says and no you are not nuts and antipsychotics are the newest rage. I wouldn't take one will stick with benzos. Don't need weight gain and chance of diabetes since it runs in family. This is me though. Phillipa

Hi Phillipa, yes I agree, no antipsychotics for me. I'm sure they have a place in mental health and truly help those with degrees of psychosis.
I've heard they're being used quite often as an adjunct to antidepressants, but I am not willing to go there. God knows they have a long way to go with existing antidepressants and how they potentialy work??
I'm not about to venture into another class of medication on a hope it "may" help, given current failures with proven augmentation strategies.
Too many "cooks in the kitchen" as far as I'm concerned! And it seems all the cooks have yet to bake the perfect cake? Not surprising given every recipe contains ingredients from every corner of the kitchen.
Geez, I'm starting to sound like I need an abilify-pie. ;-)
