METHADONE + GRAPEFRUIT JUICE (AND OTHERS)

So guys! Does anyone know if grapefruit juice INCREASES or DECREASES the effects of methadone?

What about provigil?

What happens if you take methadone WITH provigil?

What about lamictal? Does that do anything?

I am prescribed lamictal and provigil and recently started methadone for...pain. I really want to know if they interact with each other, and also how they interact when grapefruit is thrown into the picture.

I'm not positive, but grapefruit juice often decreases the effectiveness of a lot of drugs or has no effect - nothing I've heard about increasing effectiveness.

Provigil is a weak inducer of CYP3A4. Since methadone is metabolized by CYP3A, Provigil should somewhat decrease methadone levels. I have read literature of Provigil being used with opioids to counter the side effects of sedation, and there seems to be no clinically important interaction. I use buprenorphine, which is also metabolized by CYP3A, as well as Provigil, and the two seem OK together for me.

Robbie-
only white grape fruit juice will increase your levels of methadone and also antacids will also do the same. google addiction forum for a list of meds the plus and minus methadone levels. This website is owned by one of the drug companies of methadone.

Modfanil induces CYP 3A4 which metabolizes methadone. Using modafinil on a regular basis will cause methadone to be metabolized much more regularly.

In my experience, 110 mg methadone HCl/day would last long enough that I could miss a day's dose without much problem.
After five or six days of 400 mg modafinil/day, methadone would wear off so quickly that I would be sick within 24 hours due to methadone abstinence syndrome.

Sorry, I said that using modafinil regulary would case methadone to be metabolized much more "regularly" but I meant much more "quickly."

Methadone is a strong CYP 3A4 inducer.
