What is the best treatment for mixed state?

Is this a mixed bipolar state? Or an agitated depression cause that is what I must have too. The high anxiety and fears and then crash. Is that like you? Love Phillipa

Hi,
I have heard the best treatment is depakote, divalproex acid...

And if severe, divalproex plus an atypical antipsychotic...

many people find ADs to help with anxiety in a way that benzos and/or mood stabilizers(which mke me whacko)

"mixed state is a condition during which symptoms of mania and depression occur simultaneously (e.g., agitation, anxiety, fatigue, guilt, impulsiveness, irritability, morbid and/or suicidal ideation, panic, paranoia, pressured speech and rage). Typical examples include tearfulness during a manic episode or racing thoughts during a depressive episode. One may also feel incredibly frustrated in this state, since one may feel like a failure and at the same time have a flight of ideas."

I think it's an under-diagnosed condition... many depressed people don't realize they're bipolar mixed state since their symptoms are predominantly depressive. When they're agitated they still feel depressed.

I've found that SSRI/NSRI aggrevates anxiety and agitation. I also get problems with insomnia and short term memory.

I understand agitated depression, it sucks big time. (miserable turmoil for me)

My personal experience doesn't fit what is "suppose" to work. My trial on Topamax made me extremely agitated- which my dr. said he had never heard of happening because it is suppose to be one of the best drugs for agitation (but has some bad side-effects).

I think Zyprexa and Seroquel (antipsychotics) are at the top of the treatment list to calm the agitation (but not for me). Because it is considered to be part of the bipolar spectrum they suggest bipolar meds.

I've strangely switched gears recently to mostly anxiety & depression (after years of just agitated depression). I take 2mg of Xanax XR which helps the anxiety, and probably the agitation as well. (I also take 25 lamictal, a little celexa & synthroid), but I'm definitely not cured.

Ya, I went to the hospital about a year ago after trying Adderall & Abilify and went really down hill. (Adderall made me feel great initially.)

At the hospital I started seeing the psychiatrist I am now who first had me try lithium (I think by itself). I was only on it for about 2 weeks but I was very agitated & depressed. I then tried Cymbalta and I didn't seem to be as bad. Lithium is something I would try again, but am hesitant.

(as a side note, after a couple of weeks on Cymbalta, even though I had improved a little, I was still so desperate that I tried ECT. After only 5 treatments I did a 180 and felt better than I have ever felt. Unfortunately it didn't last, but it helped pull me out of the deep.)

I have also tried Depakote, which I will not try again. That was bad news for me.

Have you tried Lithium?

I am using lithium orotate 20 mg

I find it mood stabilizing and antidepressive.
I can actually feel my right cerebral hemisphere healing. It's like things are put in order and it clears brain fog. The only bad thing... it makes me a little disconnected or shut off emotionally.

I'm thinking of upgrading to regular lithium treatment. I actually went to a regular MD but he said she had no right to prescribe it :-( so I need a psych to get it...

I'm not sure I would tolerate higher dose though since 20 mg makes my mouth dry.
