What if I didn't wait long enough

Posted by Maxime on April 4, 2006, at 14:43:27

Hi

I am wondering what symptoms I would have if I didn't wait long enough to let the Prozac clear from my system.

I am really not feeling very well.

Potentially severe acute adverse reaction to a serotonin agonist.
Pathophysiology

* excessive stimulation of 5-HT1A and possibly 5-HT2 receptors
* usually result of administration of serotonin agonist to patient who is already receiving a drug capable of potentiating effects of serotoninergic pathways
* may also occur as a result of:
o enhanced serotonin release (eg by cocaine or "ecstasy")
o decreased serotonin re-uptake (eg tricyclic antidepressants)
o decreased serotonin metabolism (eg monoamine oxidase inhibitors)
o excessive adminstration of serotonin precursors or agonists (trazodone, L-tryptophan)

Clinical features

Characterized by cognitive and behavioural changes, autonomic dysfunction and neuromuscular abnormalities. Usually develops within hours of administration of precipitating agent.
Cognitive & behavioural changes

* confusion
* agitation
* coma

Autonomic dysfunction

* hyperthermia
* sweating
* dilated pupils
* tachycardia
* labile BP
* diarrhoea

Neuromuscular abnormalities

* hyper-reflexia
* myoclonus
* shivering
* ataxia

Hi Maxie

What are your symptoms?

I think you should stop the Parnate and restart it after two more weeks.

I know everyone is concerned about serotonin syndrome, but I find it difficult to believe that two days at 30mg produces sufficient MAO inhibition to cause it. You should get your blood pressure checked in any event. Some people experience hypertensive episodes when they first start taking Parnate. They can even appear later in treatment in the absence of tyramine or drug interactions.

In what ways are you not feeling well?

I don't know about serotonin syndrome. I mean, the potential is obviously there. But then again, your doc probably wouldn't have allowed it if he had any doubts.

I would be slightly more inclined to think in terms of prozac withdrawals. Your body is just now being introduced to a strange new molecule, which in itself could cause some not well feelings. But in addition, there are prozac withdrawals going on too. Its withdrawals are not so sudden as other ssri's, but I know from personal experience they are real and they can be severe. When prozac was nearly out of my system, it got downright scary how aweful I felt.
Just because the maoi is holding up serotonin levels doesn't mean you aren't still going to feel withdrawals from removal of the prozac molecule.

Just a thought. I think for the majority of people, stopping any ssri regardless of what other med immediately takes its place is going to cause some very not-well feelings...flu like kind of stuff, muscle cramps, lethargy or anxiety, tremors and shaking, sleep problems, etc. All jumbled up and confused with new side effects of the maoi.

It's tough I know. To be safe, you might want to cut the maoi in half, or maybe do it once every other day, for a couple weeks. And call the doctor if you instincts tell you something is wrong.

That's an excellent thought.

I'm still wondering in what ways Maxime feels unwell.

Well I had another hypertensive episode yesterday which I needed 100 mg thorazine to calm dowm. I am not having any of those systems that Racer listed. I think it may have been from the mochachino (that's no how you spell it) I had. It has chocolate in it. That is the only thing I could have eaten to cause it.

I took the bus back home and I had to stand and I felt like I was going to pass out. I tried to feel my pulse but it was going so fast that I couldn't even count it.

Sigh. I must be more careful. Funny when I was on it before I could eat whatever I wanted!
