I've noticed that Topamax is interfering w Vicodin

Posted by picklehead on January 12, 2006, at 8:55:50

It's like the Vicodin is just not working anymore. Well, except, of course, for the drowsy side effect (I always get that no matter what :-( Has this happened to anyone else? Does anyone have any suggestions? I just started taking Topamax again for the second time yesterday. The only other thing I take is Vicodin.

Topamax is not known for affecting the P450 system to any noticeable degree, 2D6 is one of the pathways of Vicodin. You may just be experiencing a trace side effect of Topamax, and 1 day of any drug for the most part does not start inhibiting the other unless there is a severe interaction between the two. Of course hydrocodone was create d way before Topamax so we just dont know all the interactions but it seems too soon.

How long have you been taking Vicodin and at what dose? I can give you lots of first hand information on that drug.

On and off for past 8 years. I actually did read an article somewhere online (wish I had printed it) about 2 weeks ago, that says that Topamax sort of "wipes out" the dopamine in your brain therefore inhibiting the "high". So that would make sense as to why I'm not feeling that it's working, but it's definitely not taking away my back pain like it used to either. I called Ortho-McNeil (the makers of Topamax) and they just said to call my doctor and have them give me something stronger cause there probably is an interaction.

Okay, that tells me more. Vicoden and all narcotics are of course notorious for tolerance issues. I wa thinking that your current lack of response to it could just be a tolerance thing. But if you've been taking it off and on for so long with no tolerance developing and it only starting being ineffective when you started the topamax, I'd say that pretty much targets the topamax as the culprit.
