A friend of mine with bipolar disorder uses a Dr. Humphrey in Charlotte, NC. I don't know anything about him.

She seems to have double depression with soft bipolar features. She has been hypomanic only briefly when she was younger. Otherwise, she has been either dysthymic or in an episode of major depression. The major depression is well controlled with Wellbutrin. However, the chronic dysthymia remains. She is quite functional, however.
I have a difficult time drawing an impression of her doctor (Dr. Humphrey). She doesn't talk about him much, and she refuses to undergo any more drug trials until a treatment she considers novel and effective emerges. I guess she can afford to do this, as her depression is "minor".
If this doctor is not in a position to take on new patients, try to get him to please recommend another. I find that this usually increases your odds of getting a good one.
I'll just e-mailed my friend for you to get his full name. I imagine you could find him in the phone book, though. I also asked her for the names of any other psychiatrists that she deems adequate.

Tomorrow. It might be useful to have a benzo or another sleep aid in case dexamethasone causes insomnia. If you react badly to dexamethasone, Zyprexa might be useful.

I am pretty scared to swallow that first pill. I don't know what to expect. I'm currently taking Abilify, but I keep Zyprexa on hand for emergencies.

Yes, generally. I'm taking citalopram 80mg at the moment. I took 12.5mg amitriptyline about an hour ago so I need to go to bed soon!

The pharmacy forgot to fill my prescription for dexamethasone. Over the phone, they informed me that my doctor had called in the prescription. I hope they just forgot to include it in with the rest of my prescriptions. Otherwise, I will have to wait until Tuesday 10-25-2005 to start it.

I guess we'll soon find out. As tested, I am a DST non-suppressor and have high salivary cortisol.

Yes, Theo, I had stopped it. When I realized that I had felt slightly better at 1000mg, I returned to it. It doesn't help much, but even a little bit can be a lot sometimes.It is possible that Keppra has a therapeutic window of efficacy. Perhaps dosages of 2000mg and above are counterproductive. I don't know. Regardless, I am not expecting any further improvement from it.

That's what my doctor is thinking as well.
Meanwhile, after 36 hours of my first dose of dexamethasone, I am not having a very good day. I have been more vegetative and am experiencing more dysphoria than I would expect on an average day. Psychomotor retardation, loss of interest, and amotivation are somewhat worse as well.
It could just be a bad day that would have happened anyway. Who knows. Regardless of what my reaction is to dexamethasone, if it is robustly negative or positive and brief, I will be quite motivated to get mifepristone and attack the HPA axis using a cortisol receptor antagonist.

I think everyone would 'suppress' after a while. I'm hoping that when you stop the dexamethasone, your cortisol levels will be lower and you'll feel all the better for it.
While you're taking dexamethasone, you symptoms might be worse due to the dexamethasone itself. Hopefully you'll get an AD effect when you d/c it.
If the dexamethasone does work, I imagine you'll have to take it every few weeks to maintain the benefit. Just guessing.
Hope this makes sense - am feeling a bit out of it on amitriptyline.

Day three on dexamethasone 4.0mg is starting out better than day two did. I don't feel anything positive, but at least I don't feel anything negative. I hope I haven't spoken too soon.

Hi Scott,
In your case, I guess the aim of using dexamethasone is to induce mild adrenal suppression.
Corticosteroids seem to cause the most adrenal suppression when taken in the evening, or in divided doses throughout the day.
