PLEASE someone tell me what to do

Okay, so I am not doing well as you know. I emailed my pdoc and he said I could retry the Prozac and Sulsipride (sp?). He said he could get Sulsipride from the UK.

What do I do keeping in mind that if I want to return to the Parnate I will have to wait 5 weeks for the Prozac to be out of my system.

Please, I can't think for myself right now. I need you guys.

I don't know how different Prozac is from other SSRI's other than its half-life. All I mean by that is that I don't have a history of long trials of different SSRI's.

What about combining sulpiride with another SSRI? Or is it that you have tried the other SSRI's, they have all caused dysphoric reactions, other than Prozac?

It's prudent that you don't want to cut off your escape routes. I am just wondering if there is a compelling reason to choose Prozac over any other SSRI.

Hi M,

You can combine sulpiride with Parnate if you like.

Sulpiride tends to cause lactation though. Abilify might be a better AP to try. Hyperprolactinemia is much less likely with Abilify.

It's great that your pdoc is willing to get hold of drugs from other countries. There are many drugs you could try.

This is something I've been thinking about, although Dr CattleProd isn't comfortable talking about trying to get it from overseas unless we've really exhausted everything else.

My reasoning is that Walter Kaye's work on anorexia nervosa showed elevated levels of serotonin in the central nervous system, which he thinks might be part of the disorder. Maybe a selective serotonin reuptake ENHANCER would help more with the depression and anxiety symptoms in anorexics than a medication that inhibits reuptake?

Just a thought.

Also, have you tried Effexor or Cymbalta? While Cymbalta ain't by any manner of means a perfect drug for me, it has done a lot of good for me. The only problem I've had on it, after the initial adjustment phase, is the sedation/amotivation, but with Provigil and Wellbutrin (and a daily nap), it's bearable. Also, I'm about to lower the dose to 40mg from 60mg, which I hope will help. The added benefit of Cymbalta is that it does help with chronic pain.

I'm sorry you're in such a state, and hope that you find a good, long lasting answer soonest.

Racer that't tha one thing cymbalta did for me. Relieved of any body aches and pains. Didn't know this until I stopped. Fondly, Phillipa

Prozac is the only one that worked. :-)
Zoloft worked but stopped very quickly. Prozac lasted the longest and it worked. It worked for almost a whole year.

I think he is willing to try the sulpiride because he is from England and can easily obtain it. I know he wouldn't be willing to try meds other countries. Too much paper work etc. You know he isn't receiving any payment for his services to me now? I'm not in that province. I can't give him my card. If it turns out that he really does a lot for me I will Fed Ex my card to him so he can charge the Province. It's only fair. He's a special man.

Hey Racer, I can't even find information about tianepine on the net. Cymbalta doesn't seem to be available yet in Canada. It was SUPPOSE to be but according to a database that is usually really up to date, it's not. I'll phone a pharmacy.

However, I know that Cymbalta makes people drowsy and I need activating meds. Always have. I am not ruling it out as something to try though.

Effexor made me go manic several times, so that one is a no go.

My pdoc said the Sulpiride would enhance the Prozac the way Risperdal does. Sigh. I am just so fed up at this point that I don't know what to do.

Email me okay? Or I will email you. I want to talk to you ....

Okay, so I know that the anti-psychotic is going to make me lactate. Some people have said well use pads in bra etc. It's not the lactation that I care about ... it's the PAIN. I mean it's unbelievable. Um, doesn anyone think it's feasible that I could use a breast pump? Am I producing as much milk as I think I am. This is coming from someone who has never had a baby so I don't know what it's really like. But I do know that I it hurts so much that when I turn around in bed it wakes me up. That's how hard and heavy my breast get.

I know these are some weird questions .... sorry.

I emailed my pdoc and asked him to be the decision maker. We are going with Prozac and the Sulpiride. I have to accept the fact that Parnate just isn't working for me. It baffles me though. I thought MAOIs weren't suppose to poop out. I heard people take them for years when other meds haven't worked for them.

Hi Maxime,

I know some people who've benefited from Parnate, but it seems as if Nardil is the better antidepressant. I've taken Parnate in the distant past. I would say that, for me, it was more of a stimulant than an anti-depressant. It was definitely stimulating, but I never had much improvement in mood while on it. I had severe disruption of circadian rhythms on it, as well as narcolepsy-like sleep attacks during the day (apparently that is common with the newer dopamine agonists used for Parkinson's Disease). When a tiny amount of Ritalin was added to Parnate, I had some improvement in mood as well as a better sleep-wake cycle.

Hi Sarah. I want to thank you for being so helpful in my time of need. :-) You are very kind. I tried Nardil twice. Once without a proper washout of Parnate and once with. I could handle it. I was throwing up and dizzy. I tried to see if the side effects would go away, but they didn't. I wish the Nardil had worked.

When I added ritalin I became suicidal when it wore off. The SR was no better. Then I tried Adderall XR which was excellent and then it was taken off the shelves in Canada. Now it is back. When I have the money I will be take the Adderall again. For now I take Dexedrine which makes me crash but not as much as the Ritalin and I don't become as nasty either.

I am going to talk with my pdoc (phone consultation) about adding the sulprimide to the Parnate instead before trying the Prozac. It's worth a shot.

Anyhow, I will let y'all (I've always wanted to say that) what we decide to do.

I am very grateful for my friends here on PB. I'm sorry that I haven't been very helpful to others these past weeks. :-( My brain is so fuzzy.

Could you try Abilify before sulpiride? Sulpiride is unusually effective at causing hyperprolactinemia (!) whereas Abilify seems to cause hyperprolactinemia only rarely.

Here are some other meds which are available in the UK.........

Flupenthixol (Fluanxol) - an AP which can act as an AD.... tends to cause hyperprolactinemia though

Amisulpride (Solian) - related to sulpiride

Zotepine (Zoleptil) - an atypical AP

Dothiepin (Prothiaden) - a sedating tricyclic AD

Lofepramine (Gamanil) - a tricyclic AD which rarely causes drowsiness. I used to take lofepramine!

Mianserin - an antidepressant, similar to Remeron

Isocarboxazid (Marplan) - an MAOI

L-tryptophan (Optimax) - an amino acid... used to augment ADs
