Survey and request for assistance -- mood charts

Does anyone here use a mood chart of any kind regularly? Did you make it yourself, or is it a readily available chart I can get my paws on easily?

I'm hoping to find a mood chart I can start keeping, because I am so upset so easily, and I'd like to be able to chart it for pdoc and the new therapist once I start up with her. And, of course, for me, so that I can at least feel as if I'm doing *something*, however little it might be.

I write stuff down in my day planner as a general guide - "anxious", "good day" (very rare), "meltdown", then if I remember, I expand on it when I write my morning pages. (from "The Artist's Way" ) Although I do it as a catharsis, it does document my progress very well.

I would use a form if I could find one, but all the CBT stuff I've found in the "Feeling Good" book makes me feel like a school project instead of a hurting person. So I wing it and just write it down.

I had a particularly long rant from me last week on Social (Friends aint a tv show) that came directly from those morning pages.

I use an ordinary calendar, the kind with a box for each day. I get sent enough free ones from various charities! It's not much space, but I can jot down meds & doses, whether I worked out, moods/symptoms, menstrual cycle, if I'm out of town, any other relevant details. It helps for me to see it in calendar layout.
