MAOI plus TCA=strongest?

I know it is a dangerous and very risky combo. And only some TCAs can be used. But lets think in a extreme depressed person that already used lots of ADs and nothing. My question is:
Is this the strongest ADs combination? C'omon, put your opinion here (and/or where can I find more about it)

This, or perhaps MAOI + lithium
or MAO + Effexor.

The reason I say sometimes is because
often depression can also be at the
cellular level. Lithium can help
to salvage nerve cells that are on
the brink of death, and then grow
them back to normal size.

MAOI+ Effexor is absolutely contraindicated. It is too powerful a serotonin reuptake inhibitor.
