Need med advice: Depression, OCD, GAD, ADD

Posted by babs on May 27, 2003, at 18:06:17

I am currently on 60 mg Celexa, 2mg risperdal, 20 mg adderall once a day and ativan as needed. It was a good combo until recently when my obsessions started kicked up again pretty seriously. The risperdal worked well for my OCD (tried Prozac, didn't help, and Celexa does nothing for them)but it seems to be pooping out. I wanted another solution anyway because I've gained 60 pounds from it and I never needed an ADD med bfore I went on it. So here are my options:

1. Try another SSRI (like I said, Prozac and Celexa did nothing for the OCD, made me more anxious and Lexapro made me sick as a dog).

2. Try Abilify. Scared to do this though as I had a really bad experience trying to switch to geodon a few months back. A psychopharm recommended this as my next step. Said it may help with both weight gain and ADD.

3. Get off risperdal and try klonopin or buspar. Problem is ativan relaxes me physically but does not reduce the obsessions. Don't know if this would be true for any benzo or anti-anxiety med.

I see my pdoc Friday to discuss options but I wanted some feedback from all you intelligent people. My gut says try tyhe Abilify but I'm so afraid that the same thing will happen that happened on Geodon (I became incredibly anxious, had suicidal ideations, it was horrible). Thanks guys.

I' m not in exactly your situation, but I do suffer from major anxiety, depression, and ADD, and although I haven't been diagnosed with OCD, I do tend to obsess about particular problems to an extreme degree at times (plus my sister has OCD). I find Klonopin (clonazepam) very helpful, and it seems to have a real 'synergy" with the Adderall I take. Others here have noted this too.

Again, I have no experience with full-blown OCD, but it's a thought. Klonopin certainly helps many people with anxiety, and appears to be relatively side effect-free for most after some initial drowsiness, clumsiness etc. (this took me about a week to get over, and now I have no side effects at all). It reduces my obsessive worry tremendously at a pretty low dose, which I haven't had to increase after two years of daily use.

Gad, I hope I don't sound like a broken record, but what supplements are you taking? There are a number that might help.

You said that you didn't need an ADD med until you got on the Risperdal (but that one is what is helping the OCD). It sounds like SSRI's generally work for your depression, but they are somewhat lousy for OCD and they make you easily agitated. If you are hesitant about Abilify, and since you mentioned Buspar, you might simply cross-taper the Risperdal with Buspar (keeping the Celexa) and see what happens first. The Buspar might offset a lot of the SSRI-induced agitation (maybe allowing the SSRI to work on the OCD). Also, since the Adderall was needed because of the Risperdal-once the Risp. is gone-maybe you could taper down and possibly off the Adderall-which might help the OCD as well.

Larry- Your advice is always welcome. The only supplement I am taking is fish oil. Thanks, babs
