Could anyone explain why or how rapid cycling accompanies BPll so often?
Phelp's wave diagram is helpful but doesn't answer this question. Maybe I've overlooked one in his site. Has anyone found good reading?

I don't think reasons are going to be forthcoming. The literature examining the pharmacological treatment of rapid cycling is sparse and there is no clear consensus with respect to its optimal pharmacological management.
