I was looking with interest on the clinical trials site at a new substance they are trying out called AZED6765 for TRD, it acts like ketamine, but then I went to this other site and it says the below. I canot understand how they are saying they want to test it on people with TRD on one hand and then say that if you have failed 5 different classes already then you don't meet the Criteria ??? How can this be?
Patient Exclusion Criteria:
1. Patients who are treatment refractory, defined as having failed adequate courses of treatment with 5 different classes of drug
2. History of substance or alcohol abuse in the past 6 months or dependence within 1 year

While they may have some evidence that the compound will be a good option to treat TRD, they also have to make sure that they are treating depression during the trials. Someone who has failed adequate trials of 5 classes of drugs may have something masquerading as depression -- in which case, they probably won't respond to this one, either.
Most drug trials do exclude a fair number of people. Sometimes they exclude anyone who has any co-occurring disorders, so someone with depression AND anxiety will be excluded, or someone with depression and OCD, or a personality disorder. All of those muddy the water when you're testing a drug to treat depression.
It doesn't seem as though it makes sense, but if they want approval, they have to show that it works. That means giving it to people who are likely to respond to an anti-depressant, even if they're really aiming to treat TRD.
That's my understanding, and simplified some, but i hope it helps.

Thanks Racer, this is reassuring. I was thinking that even on trials for TRD they wouldn't take certain people on because they are without hope (me :-)

Im encouraged that im not the only 1 out there looking for ketamine like substances and praying they wil become future antidepressants. Have u ever tried riluzole - or even ketamine itself???!!!

No I haven't tried either Ketamine or Riluzole.
I guess I wouldn't want to try Ketamine unless it was part of a controlled trial, where I was being monitored, due to the possible side affects. Also the trouble with this trial is that you get one shot and then that's it but the promising thing I read about one study is that it seemed to help other ADs to work.
The Riluzole I would try but then that hasn't been that promising from what I've read and doesn't tarket the same NMDA receptor spots. But I guess I would try it.
Have you tried either drug?
Are you in the States, maybe you could get on a trial they seem to be recruiting people according to the clinicaltrials.gov site.
You are lucky if you are in the States as there they seem to be plouging a lot more money into research than they are here in the UK.

This is typical of med trials, they only want people who are going to have good results. I was refused a trial for rTMS because, even though it was for treatment resistant depression, they didn't want anyone who hadn't had any periods of not being depressed in the last 5 years. 5 years!-they didn't ask but I haven't had any period of not being depressed in the last 40 years. (Later, as you did, I want to Canada and paid a hefty sum for the rTMS; it didn't work). Cecilia
