EMSAM for ADHD

Does any find EMSAM working for ADHD? Just curious since it is partly metabolized to l-methamphetamine. Or on the hand is this only applicable to oral Selegiline????

It has absolutely nothing to do with the metabolizm as the amphetamine isnt even active. What EMSAM does do is block MAO-B inhibition which causes higher levels of dopamine in your frontal cortex. Thats the goal of AD/HD stimunlants also. I've thought for years EMSAM would be a great canidate for AD/HD treatment its just the development took so long and it ended up being priced originally at $450. It just wasnt good for the AD/HD market.

I recall a study of children in israel comparing ritalin and EMSAM and EMSAM was the preferred treatment. So there is some studies going on its just taking a while.

If i hadnt got dexedrine when i did EMSAM was my secondary choice.

Hi dbc :)

I believe the amphetamine is in fact active, it's just that the conversions to methampmphetamine and amphetamine from selegeline are not that great. Perhaps I'm mistaken but if methamphetamine and amphetamine are floating around in the brain they're going to be active.

Might you be able to site this study--it sounds very interesting.

Glad to hear that dexedrine is working for you. It worked amazing for me but unfortunately I couldn't tolerate it.

Even if the amphetamine were active it wouldnt be at a level that would matter anyway. As far as i know no MAOIs produce enough Mamp to impact anything.

Objectives: The aim of this study was to examine the selegiline treatment compared to methylphenidate (MPH) in children and adolescents with attention deficit hyperactivity disorder (ADHD). Method: Forty subjects, aged 6-15 years, boys and girls, who were diagnosed as having ADHD, using the criteria of the Diagnostic and Statistical Manual of Mental Disorders- Fourth Edition (DSM-IV), were randomly assigned to receive either selegiline or MPH for 60 days. Treatment outcomes were assessed using the Attention Deficit Hyperactivity Scale (ADHS) administered at baseline and on days 14, 28, 42, and 60 following the commencement of treatment. Side effects were also rated. Results: There were no significant differences between sex, age, weight, and ethnicity of participants in the 2 groups. Both groups showed a significant improvement over the 60 days of treatment resulting from the teachers' and parents' ADHS scores across the treatment. Conclusion: Following the trial, MPH did not effect greater mean improvement as a result of the parents' or teachers' ADHS scores than selegiline. Thus, selegiline appears to be effective and well tolerated for ADHD in children and adolescents.

The study does deal with selegline but of course logically EMSAM would be prefered over it in most situations aside from its cost.

Treating ADD and ADHD is one of my doctor's specialties. He has had success using Emsam to treat these conditions.

Appreciate your responses. I currently take a low dose of Adderall XR w/Zoloft. Was wondering if I could kill two birds with one stone so to speak. I would like to try EMSAM but it's hard to get my doc to prescribe "newer" meds. She's cautious in that regard. I see her Monday and I'll bring it up as an option :)

You might try reminding her that EMSAM is not actually a 'new' med (selegiline discovered in the 60s). It's been an established treatment for Parkinson's and, to a lesser extent Alzheimer's and depression for decades (since the late 70s for Parkinson's, I believe). EMSAM mainly just allows us to use an old type of drug, the tried-and-true MAOIs, while avoiding most of the tyramine risk and giving higher blood-levels of the original drug. Maybe your doc would be less averse to such a thing? It really is an old drug--discovered in 1964, I think.

There is some research relating levels of PEA to attention deficit and learning disorders.

I know that amphetamine increases PEA concentrations, as would selegiline.
