How is Lithium added to an MAOI?

Posted by Caedmon on June 4, 2006, at 21:24:21

Anyone know? I know there is some risk of serotonin syndrome. I can't imagine it's done the same way as with an SSRI or TCA, or is it?

Does a doctor go slower in intiating and increasing the dose to determine Li levels? Is a lower level utilized than what would be used with other antidepressants? (i.e. less than .6 mmol/L or whatever?) Should levels be checked more often? Or is lithium generally regarded as innocent enough to just use the regular guidelines? Does Li toxicity carry any increased danger?

Just curious.

Unfortunatly i dont think that much effort or thought is out there involving the combo, nevertheless u raise some extremly good questions since lihtium is a drug needed to be taken carefully in the first place, then the maoi inhibting its effects and how it leaves ur body has to have some type of significant effect.

Not to mention how maois potentiate most drugs.Good questions,i see some of ur posts and im sure u know ur way,so have u tried do an extensive google on the combo?

Ive used the combo, surprise huh since ive used everything else lol,but i quckly stopped as i did notice something dident feel quite right, its possable i stopped it to soon, but i dont know im more of a "listen to ur body when its telling u something is not right" then a "wait through the bad effects" kinda guy.

I can understand waiting on a drug to build up enough to a therptutic level,HOWEVER i believe if something doesent belong in ur body, then ur body will quickly tell u it doesent, and to ignore this and wait out a week or so never seemed right to me,anyway good luck sorry i dident have more informative information!

Hi Chris.

In general, Parnate + lithium has been considered to be a safe and effective combination. The risk of serotonin syndrome seems to be very low. I am not aware of any special protocol for initiating treatment with lithium while Parnate is onboard. When using lithium to augment antidepressants in treating unipolar depression, it is not necessary to use high dosages. A range of 300-600mg is considered to be effective. This range might also be effective for bipolar depression, but would be too low to treat mania. Sometimes, the addition of lithium can produce a response within three days. I don't think Parnate potentiates lithium toxicity. It probably makes sense to check the blood level occasionally just to be sure.
