Help Needed -10 hrs. til Pdoc Appt.- What To Do?

I've been taking Amisulpride four about five weeks now treating dysthymia or some sort of depression that my doctor has listed under deficit schizophrenia. I can identify with all the negative symptoms of schizophrenia but none of the positive.

I started off at 25 one week up to 50 for two weeks up to 100mg for one week and then back down to 50 for another week and now I'm back up to 100mgs for the last two days. I do plan on going higher, but I'm probably going to start augmenting right now.

Give me some ideas out there about what goes nicely with this stuff, because it is doing nothing on its own. I mean zilch. Nada. Its like a sugar pill for me. A little loss of libido and lack of sensation during orgasm, but thats it. That isn't even a good thing.

Only thing leave modafinil out of your suggestions. I've played around with it for the past month I've been on the Amisulpride, and I swear I can't even feel the drug until I hit 1000mgs. Thats five of those 200mg babies in one swallow. This does produce a calming effect for me, but the cardiovascular effects at this dose are just too much. I'm actually out of breath in a seated position from the tachycardia. It doesn't actually really feel safe either. So. Suggestions open.

Years ago I got good results with amisulpride prozac combination. The amisulpride alone wasn't great, the prozac alone wasn't great, but the two went together well for some reason. I'm not sure another ssri would have had the same effect, since prozac is different in the way it stimulates dopamine and norepinephrine.

How high would you be willing to go with the amisulpride? I'm sorry it's not helping so far.

What's up ed,
I'd be willing to go as high as I need to. I have a feeling I'm going to feel more and more flat the higher I go once the post synaptic dopamine receptors become blocked. You know the deal. But I don't feel any worsening of flatness now at 100mgs. What do you suggest? I had a f@#$king minor car accident on the way to see my pdoc and missed my appointment, so I didn't get to discuss adding a stimulant to the ami. I have another month to play around with the dose. Have you heard of individual responses to negative symptoms at > 100mgs? I remember you saying negative symptom dose range was about 50-300mgs.
Let me ask you. How high should I go before calling it quits?

Hi Tom

The recommended dose range for the negative symptoms of schizophrenia is 50-300mg. You could certainly try a higher dose. It doesn't sound like you have schizophrenia though, so I'm skeptical about the need for a high dose.

Did you ever try desipramine? I was thinking it might help. I vaguely remember that you tried nortriptyline. What was your experience?

What I meant to say it that your deficit syndrome may not be related to schizophrenia. The diagnosis of simple schizophrenia has fallen out of favour because it may not be connected to schizophrenia, at least not in cases where psychotic symptoms are absent and impairment of functioning is not profound. I am concerned that moderate/high doses of APs might make your symptoms worse.

Tried nortiptyline for about two months and reached 75mgs. It didn't seem to really do anything. It seems that sometimes drugs have absolutely no effect on me good or bad kinda like right now on the amisulpride. I can't really tell I'm taking anything. I've tried Strattera too. It was a very short trial. Couldn't eat or sleep on it, but I do remember it providing sort of a calming effect right at first. I think I only took 25mgs. I will discuss desipramine with my doctor.

I've never tried them. I think it's a good idea to do so. Don't you? Which one would you suggest to try first. Just pick one?

Yes, it could be a good idea, but not if you have a history of paranoia or other psychosis-like symptoms.

Pretty much. There are two main drugs: amphetamine/dextroamphetamine and methylphenidate/dexmethylphenidate. Each drug is available as a variety of formulations eg. controlled release.
