Is Celexa + Buspar + Serzone ok?

Hi, I have been happy with Celexa 40 mg. so far for depression and panic....now i start to feel apaty and so on.

So it think i will reduce Celexa to 20 mg, and add some Buspar to boost up a little bit,

Because of sleep problems I also wonders about Serzone.

Let say I took:

- 20 mg Celexa in morning
- 15-30 Buspar day
- 25-75 Serzone at night

Any comments please...and what would be the result for my brain receptors?

Hi Kristen

Celexa is a potent SSRI (seotonergic). Buspar is an antianxiety medication that is also it main actions on certain serotonin receptors. Serzone could be regarded as an a'atypical' SSRI in that it inhibits the reuptake of serotonin at certain serotonin receptors. THAT'S A LOT OF SEROT0NERGIC ACTIVITY YOU'RE CREATING! I would be wary of this combo of meds. If it's *apathy* that's bothering you, perhaps you should consider a drug with some noradrenergic (norepinephrine) and/or dopaminergic action(s). Efexor XR at doses above 150-300mg has serotonergic, noradrenergic and (weak) dopaminergic effects. Adding some Ritalin (or other stimulant)to you Celexa could might be a consideration. Let us know how you do ...

Kristen,

Unless you are a very high strung person, I would not at all recommend that combination. Buspar, to tell you the truth, will most likely add to the sedative effects of Celexa. I take both, and it does for me. You might as well keep the Celexa the same dose, because adding Buspar will potentiate its' effects anyway. Taking 20 mg. Celexa with 30 mg. of Buspar would be like taking at least 40 mg. Celexa by itself. Someone should correct my statement if it is wrong, but I don't believe it is. Serzone is not recommended to be taken with anything sedating, or tiring, like Xanax. So in your case, adding that to your already semi-sedated state would be a mistake. You are having trouble sleeping? How about a small dose of Klonopin, Xanax, etc.. one of the antianxiety agents that alter GABA? Too much serotonin is the result with the 3 of these medications together. Your dopamine and NE levels will be way too low. That will result in spaciness, lack of motivation, low concentration, etc... That is my advice.

In addition to the cautions already passed along to you, if you look at the Precautions section of monograph for Serzone (e.g. at www.rxlist.com), you'll see that there is a BIG metabolic interaction between Serzone (nefazodone) and BuSpar (buspirone). At typical Serzone doses, Serzone can increase the amount of BuSpar in the blood by up to 20 times!

Now, magnitude of increase in plasma level doesn't necessarily directly correlate to magnitude of increase in BuSpar effects (or side effects). Moreover, you're talking about taking far less than a "typical" dose of Serzone, and spreading out the dosing of the different meds.

Nonetheless, please be careful here! Probably best to talk to your doctor before making ANY dosage adjustments (not that all docs are necessarily attuned to all the interactions), but at the very least, first check for possible interactions on the web, or in a book.
