Subject: Warning concerning tramadol and antidepressants

I just received in the mail the notice from Ortho-McNeil detailing the revised prescribing information about tramadol.

There have been 83 domestic reports of what are described as seizures or convulsions. The seizures have been reported after the first dose, at the recommended dosages, and at higher doses. Apparently the risk of seizures is increased in patients taking concomitant medications that reduce the seizure threshold. Ortho-McNeil points out that patients taking antidepressants (tricyclics and SSRIs) are at greater risk for seizures. Ortho-McNeil did not mention Wellbutrin (bupropion), but I think that they should have.

I recently called McNeil, manufacturer of tramadol (Ultram), to clarify the warning concerning the interaction between Ultram and certain antidepressants such as fluoxetine (Prozac). They informed me that there was an increased incidence of seizures in patients taking both.
