Subject: Urinary incontinence from fluoxetine

A female pt in her 30s with OCD who had benefited from Prozac developed urinary incontinence when the dose was increased to 40 mg qam. Bethanechol 20 mg tid was a help but she began to develop headaches.

I've seen urinary incontinence and cystitis in women on fluoxetine a number of times. It must be a result of autonomic effects on sphincter control. I've had some improvement, for reasons I do not understand, with reducing fluoxetine and adding paroxetine, e.g. 20 mg fluoxetine and 20 mg paroxetine instead of 40 mg fluoxetine.
