How many clinicians prescribe sleep deprivation for depression?

I often use it for outpatients in two circumstances: (1) In the early phases of treatment for depression while I am waiting for antidepressant to kick in. (2) If people experience a transient decrease in mood after an already established remission. Often need to repeat this 2-3 times, separated by about 4 days. It usually helps, often returning pt. to full remission or providing more rapid antidepressant efficacy while waiting for more sustained action of antidepressant (starting new one, increasing dose of old one).
