Woodbury P, Growdon J, Cotman CW, Pfeiffer E, Schneider LS, Thal LJ,
A controlled trial of selegiline, alpha-tocopherol, or both as treatment
for Alzheimer's disease. The Alzheimer's Disease Cooperative Study.
New England Journal of Medicine. 336 (17): 1216-22, 1997 Apr 24.
Comments: N Engl J Med 336 (17): 1245-7, 1997 Apr 24; 337 (8): 572, discussion 573, 1997 Aug 21; 337 (8): 572-3, 1997 Aug 21; ACP J Club 127 (3): 67, 1997 Nov-Dec.
The conclusion was that "in patients with moderately severe impairment
from Alzheimer's disease, treatment with selegiline or alpha-tocopherol slows the progression of the disease."

Given the fact that vitamin E worked slightly (although not statistically)
better than selegiline in that trial, it has become the
agent of choice, mainly due to the high cost of selegiline -- approximately
$90-$110/month for 10 mg/day dosing.
In addition, the potential for side-effects with selegiline,
plus general disenchantment with it -- as a neuroprotective agent -- in
the Parkinson's crowd has made it a relatively minor player in Alzheimer's
therapy...
However... there are some intriguing yet conflicting older studies looking at
behavioral and cognitive benefits from selegiline in Alzheimer's patients:
