1. Is it safe to add valproic acid to the patient's current regimen of lithium and carbamazepine?
Yes.

2. If so, how does one initiate treatment safely? The patient is otherwise physically healthy.
The same way you would otherwise. The only special problem I've had with "triple-drug" therapy is carbamazepine increasing the metabolism of valproate in one patient. He didn't develop an adequate valproate level until I stopped the carbamazepine.
