Gabapentin for pain

Subject: Gabapentin for migraines

I have treated a woman with desipramine for several years for a combination of generalized anxiety disorder and ADD. Her neurologist started her on gabapentin for frequent migraines, and not only are the migraines greatly reduced, she also is much less anxious, less over-reactive, not having intrusive thoughts of disasters happening to her kids, less overwhelmed by multiple tasks and stimuli, etc. She never had full-blown panic attacks, just panicky feelings. She's delighted with the drug.

I saw a patient recently who had no real clear cut syndrome, but complained of anger, frustration, depressive symptoms, chronic pain, migraines. He was put on gabapentin by his neurologist for migraines and most symptoms cleared dramatically.

I have had other patients with less complex presentations than the one mentioned who also had migraines and experienced the same overall positive effect from Depakote (divalproex).

I have a patient who has bipolar disorder NOS who presented to a rheumatologist with fibromyalgia and severe major depression with significant suicidal ideation on no psychotropics. The rheumatologist prescribed Neurontin (gabapentin) 40 mg/day (by opening the capsule and putting it in liquid). The patient had a remarkable improvement in her mood and pain within 10 days. I have increased the Neurontin slowly to 200 mg, and she was doing well at last check.
