What can get me out of my head!

I've been on Trintellix for almost a month, and it hasn't done a thing. I'm going to be augmenting with low dose Abilify in a couple of days and see if that works.

But I'm also wondering if I'm using the right meds to target my condition. I'm constantly stuck in some form of altered perception, or dissociative state, and I can't get out of my head to focus on what's in front of me...! It's like a constant state of catatonic hyper-arousal that's keeping me locked inside myself and unable to concentrate on what's outside my head and body. A chronic anxious daydream of horror.

I'm wondering if there's a better med for getting me out of this damned state...Maybe Abilify? Or what? I've been like this for 2 years...

I should add, there are two things that seem to get me out of it. One is alcohol, which I've stopped using so no longer an option, or something that instills a strong emotional response that distracts me for a few minutes. I've noticed, for instance, when I go on a date to meet a new woman, I seem to be able to keep myself present and fully feel normal during the date. But as soon as I go back to ordinary circumstances I get lost in this abyss again.

Are you only on trintillex at the moment?

How do benzodizapines affect you?

Does coffee help or make things worse?

Have you tried DHA? Lithium? Magnesium?

What about ginger root?

olanzapine can be grounding for some.

Did you try fluvoxamine?

Answers in order of questions:
Yes.
They do nothing.
Caffeine helps a bit in the morning, but too much makes me anxious, thoughts racing, etc
Fish oil has never helped.
Have never tried lithium, should I?
Magnesium hasn't helped. Tried it extensively.
Ginger root hasn't helped.Tried it extensively.
Haven't tried olanzapine. Why, would it be better than Abilify? I feel Abilify might help me due to dopamine agonism which may get me out of this funk.
I actually had a great 20 year run on Luvox (100mg last few years) till my relapse 2 years ago, so I switched to Zoloft assuming Luvox had pooped out. Zoloft did nothing, so I'm trying Trintellix now, it's doing nothing. I'm almost tempted to try Luvox again, you think the year and a half break from it might make it effective again? It really helped my sleep instead of these two other drugs that are morning drugs.

I'm not sure of the dose range for Luvox, but I think 100mg is on the lower end (100-300mg a day).

Olanzapine can be good for anxiety, abilify can sometimes cause agigation (more of an antidepressant effect).

I'm not sure what your doctor is like, but luvox might be compatable with trintillex. If trintillex is not helping, I don't see why you couldn't try luvox again.

Luvox is different in that it activates the sigma receptors. This can produce anti-ocd and antipsychotic, in addition to antidepressant effects. Zoloft actually blocks the sigma receptors which can worsen psychotic depression.

Hmmm...the first 7 years I only took 25mg Luvox and still got benefit from it. But at that point in my life my mind was virgin to psyche meds which could be why it responded so readily. Also, I've wondered what dosage Luvox's sigma 1 agonism kicks in and whether that may be why I responded to it way below the serotonin based therapeutic range.

That is very expressive. I know what you mean. It's not related to rumination?

I think some psych drugs (benzos?) may make this worse, allowing for the fact that I don't really know what you mean. I spent years in a state like that. It made (for example) visiting the accountant really difficult. My vision would almost go double, I would sort of go into the sound of his words even though the meaning escaped me.

Oh! And interestingly, alcohol helped because it made my edges stronger. As did amphetamine. With that I could actually listen to what he said rather than the sound of him saying it. I wondered if at the bottom was some terrible loneliness and a desire to merge.

There is evidence to suggest that Luvox is fairly potent at this receptor.

It would certainly bind, to some extent, to the receptor at lower doses. The two actions likely work synergistically, allowing lower doses of reuptake inhibition for a therapeutic effect.

Sorta joking but dextromethorphan (aka Robitussin). That used to put me a bit outside myself. Acts on NMDA. There's an Alzheimer's drug called Nudexda that is super expensive but essentially dxm and another med that delays its breakdown.

I've heard Marplan (Maoi) is good for derealization/depersonalization but it's currently not available across the country even if.

I've had somewhat similar "issues" in years past. My recommendation...

stimulant (ritalin, focalin, perhaps even an amphetamine...just watch the dose, especially with an amphetamine + something GABA (benzodiazepine, sufficient gabapentin, perhaps lyrica) + a neuroleptic/tranquilizer at a reasonable dose (my personal preference is Abilify, but I found that Abilify goes better w/ Ritalin or Focalin than with amphetamines...odd, huh?).

so...there you have it...upper+downer+(major) tranquilizer. Its a goof ball with a neuroleptic to even it out, basically.

You can do cognitive training. Neurofeedback could teach you to get out of your head nicely. Its not exactly for free though.
