Zoloft to Trintellix. Need wash out?

Posted by Prefect on May 21, 2018, at 13:35:42

Moving to Trintellix from Zoloft. Yesterday took last zoloft dose (25mg). Do I need a washout period before starting Trintellix?

She wasn't all that sure. Trintellix is a new drug so healthcare professionals haven;t observed it much infield.

Switching from one serotonin reuptake inhibitor (SRI) to another is generally safe. Serotonin syndrome is not really a concern. I will be taking Trintellix in a month or two. If for some reason I need to switch from Trintellix to Effexor (two SRIs), my doctor will not require me to do a washout. He's pretty sharp. Otherwise, I can't see any reason why going from Zoloft to Trintellix should be a problem. I'll be interested to know what others think.

* Switching directly between SRIs and MAOIs is extremely dangerous and should be avoided. For this kind of switch, a washout period is necessary.

Also, coming from the lowest dose of Zoloft, I really can't see an issue.

You could break the trintillex in half (or dissolve it in water) and drink the first dose slowly (over the course of the day), if you're really concerned.

I've done countless crossovers from one med to another, or with herbs involved, where serotonin syndrome was a risk. The way I handled that was not with a wash out. Instead I did cross tapers - weaning off of one while titrating onto another, keeping the overall total dose low until the transfer was complete.

Prior to that I did do washouts. I found them to be overly cautious and not necessary. They were also torture because I had to suffer withdrawal symptoms which could be nastier than the actual thing I was trying to treat.

So I went from last zoloft straight to Trintellix after a couple of days; no problems. If anything Trintellix resolved the brain zaps I'd been having from zoloft withdrawal. Been on Trin for 4-5 days, we'll see where it goes.

> 10 mg

Do you plan on increasing, or is that the target dosage?

My doctor seems to feel that less is more with Trintellix. I'll be starting at 5 mg/day and then increasing to 10 mg/day.

The 10 mg hasn't made a difference so far. I'm thinking about adding low dose Abilify on top.

Good luck, Prefect.

It might be another two weeks before I can begin taking Trintellix.
