Distribution of Neureleptics theoughout day

Hi dr bob friends,

I often get paranoid between noon and my nighttime dosage. I want to redistribute Seroquel and Zyprexa without taking a higher dosage.

Hey,

I didn't see in the signature what times you are currently taking the medications.

You can certainly try different arrangements. Olanzapine has a fairly long half life. It is usually taken once a day, but it could be taken twice a day, if you are noticing a wearing off.

Seroquel has a shorter half life. But, if you are taking an SR version, you shouldn't need to take it more than once a day. You could try twice a day, again, if you're noticing it wearing off.

The olanzapine tends to be less sedating than seroquel (less histamine effect).

If you were able to sleep fine on just seroquel at night, you could try taking the olanzapine in the morning.

Not sure if you've tried naicin. It can be a useful adjunct for some people with paranoia symtpoms (say 100-200mg).

hi. do you have any EPS or TD on those doses? I did read somewhere, someone writing that she had taken 1,200mgs/seroquel for a long time. tapering was rough (predictably).

I don't know if you'd want to try it, but some people report good results w/ perphenazine, compared to the "atypicals." In the CATIE study, it did about as well as zyprexa, with a bit more EPS and far fewer metabolic complications. I just thought I'd mention it because perphenazine I think is dosed 3x daily, something like that, and it has an anti-anxiety effect for some people.

just a thought. other than that, I don't have any ideas.

No i dont have any movement trouble. I have ended up taking much of my evening dose earlier in the day. I want to spread it out over the day as evenly as possible. I think that my paranoia is caused by the large dose taken at night and then my body asks itsself where all those chemicals are that were present during the night.

Does that sound realistic? I have done this for way too long. I think that i need stable blood levels or else my body and brain will say WTF.

I need to see that i stay in touch with my group here. Lots of folks left and new ones came and my social anxiety sometimes keeps me from connecting. I am in a light version of a psychiatric hospital. There are no utterly disturbed people here. You can have normal conversation with everyone. Lots of therapy and they dont usually put people on meds.

I may sometimes say harsh things about psychmeds but please acknowledge that it is never personal. I am on your side, right now and maybe in the near future i just have different ideas about what is best for you.

Do you think you could drop one tranquilizer and switch to a higher dose of the other one? Maybe a higher dose of the Seroquel, for instance, and then distribute that over 3 doses during the day?

Other than that, I really don't have any suggestions. Do you think maybe a slight dosage reduction in the tranquilizers might help? I've read that some people have panic and anxiety problems on tranquilizers. Its worse with the old ones, but w/ high doses of the new ones, I would think it could be a problem.

Me too

Wouldnt that be a higher load on liver and kidney? Taking one instead of two APs sure would simplify things.

But it feels like the tranquilizers help anxiety at least in the short term.

Seroquel
0-0-0-450
Zyprexa
2.5-5-5-5

It works better that way and i can still sleep well.
