Emsam + other meds

Posted by pedr on September 17, 2017, at 17:12:12

I know that anything serotonergic is a no-no but what about Bupropion and abilify? Does the dose of emsam matter (i.e. <9mg or not)?
Thanks.

Depending on the dose, I have heard of selegiline being used with SSRIs.

I wouldn't think selegiline with abilify would be an issue. However, selegiline with bupropion could.

The issue with selegiline and bupropion would be excessive dopaminergic stimulation. Basically, you are shutting down two metabolic pathways for dopamine (MAO-B and DAT) so excessive stimulation and possible psychotic reactions could be possible.

Again, the interaction could be dose dependent. If you were to combine the two, I would very cautiously combine one with the other.

Thanks Linkadge. I've read that SRI's are the ones to avoid for MAOI's (mostly from the excellent psychotropical.com) but that's good to know about bupropion. TBH I wouldn't miss it that much, I'm not sure what benefit it gives me (as with most of my meds sadly).

My PDoc is very cautious around MAOI's so I suspect he's not going to allow me to take anything anyway. I just wanted to see what people's experience with this was.

That is a good approach (i.e. no other meds when taking an MAOI)

That being said, Selegiline is more selective towards MAO-B. MAO-B primarily affects dopamine.

In higher doses (as used for depression) it starts to affect MAO-A as well (which elevates serotonin).

There are a number of reports of Parkinson's patients taking an SSRI and selegiline. However, they were likely taking lower (MAO-B selective) doses of selegiline.

Wellbutrin probably could be combined with Selegiline but would have to be introduced extremely gradually, just to watch for overstimulation.

Abilify is ok to combine with Emsam. I use a small dose and it seems to help--

great, good to know. I take just 2.5mg. Anything higher makes me more depressed.

I'm a big fan of low dose Lithium. For me anyway it makes everything work better.

That's about the dose I use, although sometimes I use a little less, just on the theory that less (if it's effective) is more.

I wish Lithium liked me. I tried very low dose Lithium Orotate and it put me in an incapacitating depression pretty quickly.

I was fine until 12 mg. Then kefir and then coffee became a little sketchy. In addition, and unfortunately, there isn't a decent pain medication to be taken with it-- excluding an opiate. I'd be pretty careful-- sounds like you are. Some here have mixed their meds and know more than I do, but my experience with coffee was sobering :/

But it really did help my depression.

I hope Abilify is helping.

I find lithium carbonate has more antidepressant effects than lithium orotate. Lithium orotate made me somewhat depressed too.

wow now that is interesting. I wonder what that's all about? I had such a bad reaction to Orotate that I would be very reticent and scared of trying carbonate.

What was your bad reaction?

I read recently that lithium orotate was harmful while lithium carbonate was helpful. I don't remember the details.

a rapid and intense increase in depression. No other effects that I recall.

Interestingly, the dosages used for bipolar disorder (900-1500 mg/day) make me more depressed while low dosages (300 mg/day) act as a mild-moderate antidepressant. I can't do without it. Two additional benefits include the lengthening of chromosome telomeres and the mitigation of the risk of developing Alzheimer's Dementia.

Lithium orotate induced some form of emotional overtone. Its hard to describe, but it felt a bit like combining lithium and choline.
