Melatonin and bipolar disorder

Posted by Ben on December 3, 2016, at 4:53:35

I have bipolar II disorder sind more than 15 years and I have combo of mainly Lithium, Lamictal, Parnate, T4. I have this combo for a long time now but since some month I have problems to sleep (wake up several times). I was thinking to add melatonin but I dont know if its only helpfull to fall asleep or also for the waking ups during the night? Further I read that melatonin should be used with caution if you have bipolat disorder?

The parnate is what is causing the insomnia (likely).

You could try melatonin, but I would start with very low doses. I think melatonin is metabolized by MAO-A, so an MAOI would increase levels of melatonin.

don't think adding meletonin would effect, because parnate is a MAO-B inhibitor, which is mainly dopamine and norepinephrine .... but not sure

parnate defeintly is causing insomania, you may ask about something called Rozerm, there's also a new medication out now for insomnia called Belsomra, it's new but it does have a small potential for dependence similar to ambien but nothing major

I take to hole dose of Parnate in the morning and it stimulates me until 4 p.m. . After that I get tired so that falling asleep is not the problem (with all other meds) when when I go to bed. The problems are the several wake ups during the night. This problem took placed after I was on Parnate for a long time before.

ask about Rozerm or Belsomra, or a sleeping medication that will help your sleep maintenance (staying asleep)

I wouldn't worry too much about an interaction, in that your body already produces melatonin naturally.

The real issue with supplementation of melatonin is that individuals vary considerably in how they metabolize oral melatonin supplements. The effective dose varies by well over a factor of ten between individuals.

The dose-response curve for melatonin is not a straight rising' line. Instead, it's an inverted U-shape, appearing a lot like the 'standard curve' applied to statistics. As you increase the dose, the effect increases, then it flattens out, and finally, it falls sharply. Every person has their own U-curve, and the dose at which the peak effect occurs varies (as previously noted) by at least a factor of ten.

The very best way to do a trial of melatonin is to obtain a liquid solution of the hormone. For solutions, dose is directly proportional to the volume. Therefore, it is very easy to gradually increase the dose, and to monitor the effect. It shouldn't take more than a couple of weeks to figure out the right dose. Start low, and go slow, until you get the desired effect.

Regards,
Lar

I've taken all those medications at some point. And I'm currently on ketamine, Lithium, Parnate, T3 and T4. All the medications you are taking, in my experience anyway, produce insomnia. Especially the Parnate. I use a sh*t ton of Melatonin and I have bipolar. It might as well be placebo but I do it anyway. Bought 5 grams bulk powder off ebay and I just take like a key bump every night and wash it down with juice. I don't even bother to measure it anymore but I probably take something like 50 mg nightly. It does jack.

Sleep can be a bitch with Parnate. When I was on 80 mg the only thing that could touch the insomnia was 60 mg of Dalmane. Which is a lot, but I got a deep, dreamless, uninterrupted sleep. The nice thing about Dalmane is that it's got an active metabolite with a very long half life so should you ever want to quit, it will be basically self tapering.

But other than that Seroquel 50-100 mg can be pretty effective for sleep. Or 100 mg of Benadryl. Although For some reason the sedation of Benadryl goes away after one day of taking it (for me anyway) so I can only use it about twice a week.

There is no evidence to support the claim that melatonin supplementation assists with sleep in any way.

I found melatonin would put me too sleep earlier on when I was starting parnate, but when the parnate started to work, the melatonin would give me SERIOUS cognitive and other side effects the next day.

I don't remember the biochemistry behind it but melatonin is in the serotonin metabolism chain so it would make sense that there would be a cross reaction with a MAOI
