I wrote a post a couple of weeks ago about how I felt I had exhausted my options, and was suffering from a strange mix of severe agitation/dysphoria, alongside dysautonomia and akathisia.

I'm currently in a public psychiatric ward as a voluntary patient, and am also being reviewed by a neurological team.

The neurologist did observe my walking and said it looked fine to her, so go figure -- I still have a really bizarre medley of symptoms like feeling pulled down by gravity, feeling off balance when I walk (forcing to me shuffle or limp), feeling like I'm in a boat etc.

I asked the neurologist for a tilt table test but she said no. They're doing an MRI and EEG.

I've met with the psychiatrist -- a professor -- who has suggested 6 courses of ECT.

It's a difficult decision because as much as the medical team are implying my symptoms are psychosomatic, how can they possibly guarantee ECT won't worsen things.

On the other hand, every minute I'm awake is total psychic agony. I am allowed to leave and be seen by neurology as an outpatient. My parents have also taken out private insurance, which means in 7 weeks I can go to a decent clinic and try rTMS. However, I know for certain that I won't survive for that long -- the suicidal ideation has become too overwhelming.

So I will most likely start ECT Monday and just hope for the best, I.e. that it doesn't make anything worse, and prolongs my life long enough to go inpatient somewhere else in August.

It is written here, [...every minuet that I am awake is total psychic agony...the suicidal ideation..overwhelming....
Here the tragedy of this site that allows you to advocate drugs that could cause what you see here, is a travesty of justice. And worse, some of you will want to defame me and the Jews that could cause others to discard what I write here that could have prevented this tragic soul from having to want to kill themselves. Oh, the horror of this site.

Continuing with the ECT might be a reasonable option.

I assume you've tried lithium - perhaps in combination with imipramine?

Have you tried Mirapex? It has shown some promise for certain forms of resistant depression.

How are your sleep patterns?

You get delusions from menatl illness, i do all the time, because when i feel like i'm going to die, i can't make the connection between that and anxiety.
The brains defense mechanism is to dissociate and start thinking about other possibilities like a neurological disease.

I am at the ER 2-5 times a month checking my heart because i'm sure i have an heart attack.
Turns out every time that it is anxiety, cause all the tests ALWAYS comes out fine.

But anyway i go there 5 days later thinking the same thing.

It is anxiety and other mental illnesses, it's nothing to joke about.

It's EXACTLY the same thing with your delusions Lou.

You have to search your heart now and stop this, cause your soul is going to die because of all the worry in your heart.

Stop obsessing about jews and mr hiusung.. You know deep inside that it is just delusions.

Your a good guy Lou!

i've been in horrible dysphoric states, to the point where i don't want to do anything, i have energy but i get a very unpleasant feeling when im doing something, like reading, or browsing the web, ill just go into a state where i'm irrtible, ill be in a room full of people watching a football game on sunday and my mood will shift into a extremely dyphoric mood, where i don't be around

the thing is im not depressed im just in a agitatged bad mood, and everything feels dreadful and on edge

ECT im not sure will help that, i do know it helps depression but not agitated moods, but hey it may work......

anxiety medications usally will help dyphoric moods, paticulary benzo's or lyrica

Nope, never tried Lithium -- I'd be reluctant given its anti-thyroid and anti-dopaminergic properties (a lot of my symptoms overlap with hypothyroidism).

I haven't tried Mirapex. Not really sure how many of my symptoms are even straight depression anymore.

Sleep patterns aren't great. Usually awake 2pm -- 4am. But I've been a night owl most of my adult life.

Lithium really isn't antidopaminergic as much as it is a dopamine system stabilizer.

I was resisting any form of bipolar diagnosis, but am quite pleased with how a low dose of lithium has improved my life. Lithium has been like a dimmer switch on a set of high intensity flood lights shining right at me.

Anyhow, you say your symptoms overlap with hypothyroidism. What have your top three symptoms been over the past 6 months?

I assume you have had your thyroid checked?

If there is any kind of bipolarity or cycling, then a mood stabilizer could bring relief.

The thing is that i'm not that good at english but this is something i need to understand, cause i also think some things on this forum don't add up.

One thing i've noticed but never wanted to take up, cause i'm scared of being taken as psychotic and delusional, is that whenever i seek the internet for medications or diagnoses, 2-3 hours later there is a post about it on this site.

Am i interpreting it wrong or is this forum a test that was done in the mid 90s, to test the, then new, thing called ELIZA. A computer program that was built on an algoritm to sound like a human being and answer things correct.

At the time when ELIZA came, then there was a lot of people thinking that this could be the new thing in therapy as more and more people gets sick.
And if a computer could do the job instead of a person, then it would be more effective and could help more people.

ELIZA didn't became a succes, but i have thinking about this for a long long time now, because some people here respond to things i just have googled, but not asked here..

I don't mean that it is ELIZA, but something like that.

That this is a test to se if it works.

I have critical thinking and i'm not psychotic, it's ruled out many times. But in some way, sometimes, i feel like this site is encouraging me to start taking medications, based on my google searches.

I can be wrong, and i can be dumb as f**k for wanting to discuss with Lou pilder, but i want to hear what he has to say.

Lou, will you write me a email and explain this site to me in the way that you feel that it is, and try to do it in an easy way so i understand?


If you say yes to this question, i will post my email on this site.


And for all the others here, i don't think that you are robots, but some things don't add up for me.
They just don't.

And for Lou. If you are right, i will take back everything i've said.
And if you're wrong. Well then i don't think this community will accept me anymore, but that's a risk i will take.

Cause i have a bad feeling about this and have had it since i started to post here.

Hmm, it's tough to narrow it down to three, but I would say:

Akathisia
Constant hunger and hypoglycemia attacks (I never feel 'full').
Feeling off balance when I walk
Inability to tolerate any kind of physical exertion (I 'crash' and experience dizziness, heart racing etc.)

This is all superimposed over my regular anxiety and depression.

Yes. My TSH is 3.3. I've read that may be high enough to cause symptoms. However, I couldn't tolerate thyroid replacement.

One doctor told me a virus (in the context of CFS) can knock out your thyroid, so I don't know if the problem is located elsewhere.

It can matter which type of thyroid hormone one uses:

T3 (Cytomel) made my depression very much worse. I think I might have been more anxious also, but I don't remember so clearly.

T4 (Synthroid) made my depression mildly better without any adverse effects.

3.3 mIU/L is high for TSH. You might not do well until it is brought down to between 0.5 and 1.5.

What is "replacement" as opposed to "augmentation"? The dosages of thyroid hormone used in depression as augmentation can be higher than those used to treat hypothyroidism. I think T3 was chosen first to be studied in depression, possibly because it is biologically more potent than T4. However, it would be a mistake to consider T3 to be simply a more potent version of T4. Being chosen first, T3 was extensively studied. T4 was mostly neglected, even though it can be very effective.

Combining a TCA (imipramine) with T3 was the treatment chosen to experiment with. SSRIs were not available at the time. Still, noradrenergic agents might be ideal to augment with thyroid. Desipramine can be a better choice than imipramine for producing NE reuptake inhibition, while thyroid makes NE receptors more sensitive. I imagine they would work synergistically. Desipramine is more selective than imipramine for NE and produces milder side effects, particularly those that are anticholinergic.

Another drug first studied for combination with TCA was lithium. If you are heading in the direction of trying a TCA, you can first add thyroid and then add low-dose (300-600 mg/day) lithium to be taken at the same time if you don't respond well enough to the thyroid (assuming that you can tolerate it).

I tried Cytomel and Dessicated Thyroid (T3 and T4).

Both felt like too much for my system and 'crashed' me, worsening my vertigo, fatigue etc. I have read that adequate cortisol levels are necessary to tolerate thyroid replacement. A saliva cortisol test revealed that mine were low. However, as I've said, it's difficult to know where the problem is stemming from. I would have thought that if it were straight depression, then Reboxetine and a couple of other antidepressants tried during this period would have not made me feel profoundly physically worse.

I agree but doctors seem to be less open-minded and go by the standard range (0.0-5.0) here.

I have read that in chronic-fatigue-like syndromes, the HPA switches off as an adaptive mechanism .. And that sufferers become far more sensitive to crashing from medication. I am really not trying to be difficult because I wish it were straight-up depression and that I could tolerate an AD. But I feel so physically unwell that that seems unrealistic.

ggg Unit that us the old range for TSH here. The range Scott gave is what USA endocrinologists use. And mine gets upset and changes doses if gets higher than 1.5 which it hasn't in years. Phillipa

Why don't more people simply ignore trolls rather than entertain them with replies? After all of this time, it should be obvious that the frequency with which a troll is replied to or challenged, the more he will post.

Please rewrite the subject line should it be hijacked so as to preserve the relevancy of this forum. I know this is a nuisance, but I think it is worth the extra effort.

For making it more clear than it already is :
I'm trying to see if he can talk about other things.

I't basic psychology..

And in this case it didn't work.

But it was worth a try.

Jonhed sincerely SLS is a great guy with his own psychiatric problems and does take medications. One of the strongest people here. One that should be taken seriously as he truly cares. Phillipa
