Partial response to Zoloft?

Posted by Meatwood_Flack on July 19, 2012, at 21:04:31

I went through an unsuccessful trial of Zoloft (generic) a few months ago, but experienced something strange when I reached 50 mg. For two consecutive days each week for three weeks (six days total), once late afternoon rolled around, my mood lifted considerably, if not completely, and much more than the small lift in mood that occurs later each evening than the aforementioned episodes. Would those constitute partial response even though I never experienced it again (The last time it happened, I was at pdocs office and she bumped me up to 100 mg)? This last occurence was also shorter lived, only a couple of hours. Has this sort of thing happened to anyone else? Is it a clue that might help further treatment choices?

How long total have you been on zoloft? 50mg is a starter dose I think 200mg is the top dose. I think I'd stick with it a while. Could be a sign that the med may work. Have you been on other SSRI's in the past? Phillipa

I think I was at 4 or 5 weeks at 150 mg when I bailed on it. Pdoc wasn't happy about that, but when I told her about the afternoons of feeling better and asked if she thought I'd had a partial response to the med, she said she didn't know. I went to effexor next because I had read that SSRIs were not effective for melancholic depression. Maybe I jumped the gun.....

It is very common in depressive disorders for mood to be very bad in the morning but lift in late afternoon or evening. However, it might be a sign that the Zoloft is working a little. Zoloft was a very good drug for me for a while. 50 mg is a very low dose; upping it might give it more of a chance to work.

What's irritating is that, over the month afterward that I stayed on it, there were no more such episodes. Dosage was bumped up to 150 during that time.

I've recently started Zoloft, though I titrated up to 200 MG rather rapidly.

According to another site, Zoloft typically reaches peak plasma levels in about 8 hours, unless you take it with food and then it is 5.5 hours - the other site also reports a 25% increase in peak plasma levels when taken with food.

How do those times line up with your late afternoon mood lift? It sounds to me like bumping up your dose could be useful..
