(Health.com) -- The growing use of a popular drug in the long-term treatment of bipolar disorder is based largely on a single, flawed clinical trial that may be steering doctors and patients away from drugs with a more established track record, a new review published this week in the journal "PLoS Medicine" suggests.

The drug, Abilify (aripiprazole), was initially used to treat schizophrenia and acute episodes of mania, the better-than-good feeling that characterizes bipolar disorder. In 2005, the Food and Drug Administration also approved it for long-term use in bipolar patients whose moods have stabilized (known as "maintenance" therapy).

Since then Abilify sales have more than doubled, according to market research cited in the review. And a 2008 survey of doctors found that it had become the first-choice drug for bipolar maintenance therapy among 23% of psychiatrists and 16% of primary care physicians.


The medical research does not appear to justify the widespread use of Abilify for maintenance therapy, says psychiatrist Alexander C. Tsai, M.D., one of the lead authors of the review and a visiting researcher at Harvard University. "We failed to find sufficient data to support its use."

Tsai emphasizes that individuals taking Abilify for bipolar disorder should not abruptly stop the medication. "It may actually be working for some people," he says. "But it's certainly worth talking to your doctor about how it's working for you."

After noticing that a growing number of patients seemed to be asking for Abilify by name, Tsai and his colleagues performed an in-depth search of double-blind, randomized controlled trials -- both published and unpublished -- on the use of Abilify for the long-term treatment of bipolar disorder. Studies of that design are considered the gold standard of medical research.

Their search turned up just one clinical trial, data from which was used in two published studies. The trial was funded jointly by Otsuka Pharmaceutical Co., the Japanese company that developed Abilify, and Bristol-Myers Squibb, which markets the drug in the U.S.

That trial had several important limitations, according to Tsai and his coauthors. For one, they say, it may have been too short to judge the drug's true effectiveness in preventing mood swings over the long term. The first phase of the trial lasted for 26 weeks, and less than one-fifth of the participants went on to complete the 48-week follow-up phase.

What's more, the study population was limited to people whose mood had already stabilized on Abilify during the preliminary phase of the trial, and the placebo group therefore consisted of patients who were abruptly switched from Abilify to placebo. The potentially harmful effects of rapid drug withdrawal may have made the placebo group appear artificially prone to relapse -- the key finding that made Abilify appear beneficial, Tsai and his colleagues say.


Psychiatrists have continued to refer to the results of the trial despite these weaknesses, Tsai says. "Frankly, I think it's an embarrassment to the profession that [Abilify] has been accepted so uncritically for this indication."

Gregory E. Simon, M.D., a psychiatrist at Group Health Research Institute, in Seattle, says that the perceived flaws noted in the review are not so clear-cut. "The methods of studying long-term effectiveness are complex, and experts disagree regarding the best ways to study this question," he says.

Alternatives within the same class of drugs, known as atypical antipsychotics, include Seroquel (quetiapine) and Zyprexa (olanzapine). Older drugs, such as lithium, also remain in popular use.

Evidence for the long-term effectiveness of lithium is far stronger than that for newer medications, including Abilify, Simon says. For that reason, lithium remains his first-line treatment for maintenance therapy in bipolar disorder, despite the potentially dangerous side effects associated with too-high doses.


The increased use of Abilify for maintenance therapy may be leading to higher treatment costs for consumers, Tsai says. Abilify runs upwards of $600 for a month's supply and is not yet available in a generic form. "But our main concern," he adds, "is that patients are getting diverted from more effective treatments."

Sonia Choi, the director of public affairs for Bristol-Myers Squibb, said in a statement that the company has conducted five long-term studies of Abilify in the maintenance of bipolar disorder, some of which were presented at medical conferences rather than published in peer-reviewed journals. (Tsai and his colleagues restricted their analysis to peer-reviewed randomized controlled trials.)

Bristol-Myers Squibb and Otsuka are committed to making data from those studies available to physicians, Choi said. "We are confident in the Abilify clinical development program and will continue to make available information about the efficacy and safety of Abilify from our studies in this and other indicated uses."

Sandy Walsh, an FDA spokesperson, said the agency will review the new study.

Interesting article. Thanks herpills. Abilify has been advertised on television and in very glossy magazine spreads. I haven't seen them for awhile. The ads show a gleaming, robust young man dressed as a welder. Abilify allows him to do his art, touching upon a sensitive spot, I imagine, that treatment blunts creativity.

Why one med gets taken up and favored over another interests me. I'm speaking abstractly. If abilify is working--keep it going. It was presented to me by my own doctor had having a novel action.

Did you know motrin, tylenol and ASA now are seen as factors in heart attacks and strokes. Pretty sad that seems so many meds have horrible side effects the one just cited was not for long term use sometimes single time dose post heart attacks so now what suffer with headaches also. Phillipa

Hi Phillipa, most meds have risk factors. Maybe consumers have been slow to recognize this. Even foods can have risk factors to certain individuals. Cosmetics, topical agents, too. Shampoos and lotions absorbed through the skin that affect susceptible individuals.

I think the way abilify was marketed is interesting--the way it was presented to medical practitioners (a type of consumer /distributor), and to the general public.

The article doesn't really mention specific risks, does it? I might have overlooked that part.

As a long term Abilify patient, I find this a bit disturbing. Shrinks seem to love Abilify. I like Abilify because its the least mind-numbing antipsychotic I've found. I guess I'm kind of lucky that a lot of my symptoms overlap with Schizophrenia; Abilify has been shown quite effective in Schizophrenia (thought not as effective as Zyprexa, apparently),and it has minimized my agitation+psychosis w/o many (any?) side effects. Mood swings...not so much. I had to add an antidepressant for psychotic depression, but then again...polypharmacy seems to be the rule in Bipolar-land.

CE, I love your thread title. About sums up the love/hate meds thing. I'm very glad Abilify is working for you. And your poly-pharmacy is more like dual-pharmacy :-) That's good. Abilify is really potent--and that's a good new tool. I could actually tolerate taking it whereas seroquel would freak me out. Ultimately, Abilify wasn't a helper for me.

I think life is to be lived while we have it. Wisely spent. If it takes a medication, well, honestly, I'm not thrilled. I read up about risks, then make a choice. I'm glad there is medicine. I'm glad that you can take a little pill and walk around and be alright w/o needing to be oversedated.

At a certain point, I have had to put my apprehensions to rest. I take xanax--over ten years now. I don't scare myself by going to the Ashton site anymore. I know chronic benzo use isn't a health tonic, but neither is unabating anxiety.

You rock, imho.

That was my experience with it. Didn't work, then went off it and felt worse. And had to go on more medications than before the abilify.

Thank you for posting about the questioning of the long-term use of the drug called Abilify.
You see, there is a great amount of information that if people were to know of it {before} they take these mind-altering drugs, that there could be a more informed person so that the decision to take these drugs or not could be determined by the taker with more understanding as to the long-term or short-term consequences to them if they take the chemical into their system.
Now how many people that take Abilify know what chemicals are in the pill that they take into their system? Does the pill prescriber tell them before they go to the pharmacy and have the pills bottled for them?

Now It did not stop in 1945 but was continued and to understand this I am not permitted by the owner of this site to post here about that. But you could do your own search about {operation paperclip} or [I.G. Farben} or {Rockefeller and mass murder} or just {psychiatry and the holocaust} or {T4}.

And I wonder what he means by not posting {where} you could find this information. Here I am not posting {where} you could find it as to go to a particular web site or such. All of this that I am posting here I can be tie in to show you what has been {goin' on} for thousands of years and now the liturature states that about 42,000 people died from psychiatric drugs last year and that the stats show a geometric progression to lead to that in the future if people are continued to be given these drugs to increase geometrically the number of deaths from the drugs. How many deaths from these drugs will it take before (redacted by responndent) can be seen? And how many times must {redacted by respondent} before one is {redacted by respondent}? There are risks in all we do pesticides on foods etc. To live in fear of all doesn't in my opinion only lead to a life that has quality. I feel do what you need to do for feeling well. You could cross the street and be hit by a car. Risks even in bearing children. Do what is best for you. Phillipa

In my research for the chemical composition of Abilify, I am trying to confirm as to if there is or is not naphthalene in the chemical composition of the drug. I based this initally on what I thought was correct from my study of ergots and fungi in relation to mind-altering drugs used by ancient sorcerers and how that knowledge has been used in the synthesis of psychotropic drugs. I need to confirm this and will post back later with as to if this is a constituant of the chemical make up of the drug or not.
In regards to what the drug can do to the one taking it, there is the FDA warning of incresed suicide thinking.

The aspect of that Abilify has in its chemical make-up naphthalene. let me clarify this.
In the following site, naphthalene is listed as a {key hotword}. But I think that the naphthalene is not part of the structure chemically of Abilify, but perhaps part of the chemical structure of the process of making the tablet. If that is so, then as to if naphthalene is a chemical that goes into thhe system or not of the one that takes it, I do not know.
Here is the site and you can see at the top under {hot keywords} that naphthalene is mentioned. The site does not say as to why it is a keyword thatI can find.

The question at hand is what is in the chemical compostion of {Abilify}. In my research concerning the use of mind-altering drugs in the history of those that used them to alter the minds of others to control them or kill them or to supply halluncinogenic drugs in {religious practices}, I found much concerning the use of fungi, mushrooms, ergots, peppers and such that went into the drugs that the ancients used. In the midevil period and beyond, coal-tar and petrol extracts were used such as benzene and naphthalene.
Now in relation to Abilify, I said that there was Naphthalene in the chemicals in that drug. What I now have seen is that the two benzene rings fused in the chemical structure of Abilify is what Naphthallene is an analog of. That is called {quinolinone}.
Here is a link that shows the chemical structure and you can see the two hexagons fused.
Then you can also see that there is {piperazin} in the chemical structure. That is a synthetic that is close to the natural drug from a peppper plant used by the ancient {sorcerers} to alter the mind of others.
The construction of ABILIFY then, has the potential to produce a mind-altered state from these chemicals. Sadly, this drug can kill you and I have the list of those that commited suicide while on this drug and the list of those that died while on the drug. These are public records.

I can no longer watch the suffering go on here by those ibited hhere by the owner of this site from posting whhat IMHHO could save your life or keep you from getting a life-ruining condition.
The chemical inn quedtion here is made up of chemicals that chemist alreadyy know have mind-aaltering ability. This Abilify has a chemical structure that I have shown just part of and hhave not yet posted about {piperazin}which with te other coal-tar compounds in the drug could cause a blood disease along with being a {nerve agent} that could cause Tardive Dyskinesia and Neuroleptic Malignant Syndrom, sexual dysfunction, and a lot off other adverse reactions, and death.
