Posted by Phillipa on June 10, 2009, at 15:58:32

Seems this lady in the store we purchase a lot of our items for ebay is losing weight and seeing her doc he put her on prozac 20mg doesn't feel much better but working and acting fine. But she's skin and bones keeps losing weight. Doc has tested thyroid. Any ideas why? Love Phillipa

It can be complicated to find out especially in the elderly? My first thought is cancer or cardiac/pulmonary problems because that's what I see mostly. But then I found this nifty mnemonic online geared toward weight loss in the elderly. I think some of it could apply to any age group. I hope this helps your friend.

M Medication effects
E Emotional problems, especially depression
A Anorexia nervosa, alcoholism
L Late-life paranoia
S Swallowing disorders
O Oral factors (e.g., poorly fitting dentures, caries)
N No money
W Wandering and other dementia-related behaviors
H Hyperthyroidism, hypothyroidism, hyperparathyroidism, hypoadrenalism
E Enteric problems (GI problems)
E Eating problems (e.g., inability to feed self)
L Low-salt, low-cholesterol diet
S Stones, social problems (e.g., isolation, inability to obtain preferred foods)

Oh she's not old only in 40's. She says she just doesn't feel hungry. But she's eating more than I do. Phillipa

My wife lost seven pounds on prozac in one week and stopped. Prozac is like speed for some people and they lose their appetite.

prozac often causes weight loss, probably something in how it interacts with 5ht2 receptors

could try a different ssri or add a small amount of mirtazipine or cannabis

I think the answer is surprisingly simple: it's just a matter of time before she will start to gain weight. Any SSRI can work as an appetite suppressor and make you lose weight; it happened to me on Paxil (paroxetine). But this effect is TEMPORARY. That's why the SSRIs have never been approved as diet drugs. Prozac has such a ridiculous long half-life (seven days), that both initial side-effects and therapeutic benefit of serotonin reuptake inhibition might take forever to subside/manifest. But take my word for it, she WILL gain weight provided she takes the Prozac long enough. For some people, it takes more than SIX MONTHS before they start to gain weight. But on any SSRI, you WILL gain weight within a year, mostly within half a year.

Exactly. 10mg of Paxil is not an effective dose for significant SRI (serotonin reuptake inhibition). No significant SRI = no weight gain.

Well, that's not true. But that must have been the reason. Other mechanisms than SRI can cause weight gain, of course. Paxil has antihistaminergic properties which are likely to induce weight gain. But 10mg is a rather small dose.
