I know this is a recurring issue on this Board and noticed this while reviewing the newest Medscape Psychiatry this AM--It is a Continuing Medical Education presentation on current strategies for treatment resistant depression--medscape reguires registration but it is free. Thanks fca

Great articles. Thanks for sharing.

I don't believe in the term 'treatment resistant depression'. It is just a polite way for the doctor to admit he/she has misdiagnosed the cause of the depression and chosen the wrong drugs, and in doing so points the finger of fault at the patient's supposed resistance rather than at their own flawed guidance. They do the best they can I'm sure, but the brain is a mystery. I think too often they assume depression is limited to just serotonin or norepinephrine levels.

Does this mean that the right drugs exist for most people?

Maybe someday-say a hundred years from now-the right drugs will exist for most people, but they sure don't exist now. It makes me angry- all the articles talking about remission vs response; plenty of us would be more than happy just for some response, forget remission. What fantasy world are these doctors living in? Perhaps one financed by the Effexor and Cymbalta manufacturers? Cecilia

I think of treatment resistant to mean the people who do not respond to drugs or treatments as readily as some other people.
Some people, (even me in the past), respond to one out of the first handful of treatments offered. Others try and try and try lots of treatments with little relief. I don't necessarily think that's the fault of the doctor's lack of skill. A good doctor may be able to zoom in to good hunches, ie noting that atypical style depression may repond to an MAOI, patient doesn't do well with SSRIs, looking at thyroid, ruling out a sleep disorder or physical disease- but to talk to someone for an hour or so once in awhile and from that to know exactly which med to use sounds like it's asking for psychic skills. Depression and other mood disorders can be complicated, and so far there is no litmus test to indicate precisely the cause or best treatment for a particular individual. I think researchers are working on it.
