MAOIS- Car accident , surgery tomorrow ,quick !!!

hello
just asking here if there could be a problem in my friday surgery . I have 2 broken bones on my
right leg due to an accident car . Still alive
dont know how , it was a tremendous impact.
I take Nardil , so Im very worried about the surgery .
I dont have much time to decide something
Please tell me something
what shoul I do?
SHoul i stop taking nardil rigth now?
wich kind of anestesia they should use?
They must know , but I want to be sure , I dont trust in some doctors around here.

help Urgent !!!

There are certain types of aneasthetic they can use with MAOIs. Just tell the doctors what you are on. Everything will be alright.

Yeah, well. . .if everything were only that simple. I'm sorry to be an alarmist, but in instances of emergency surgery for someone whose taking an MAOI, that anaesthetist has to be absolutely sure he/she knows what an MAOI is, how it affects blood pressure, the contraindications, etc. etc. There's simply no room for guesswork. The anaesthetist has to know for sure what compounds are safe, and to closely monitor blood pressure throughout the procedure.

I agree. I think you should persuade your doctor to inform the anaesthesiologist in advance so that he can do his homework.

I guess this post is too late.

Just make sure the doctors know that you're taking Nardil, and if possible, have a talk with the anesthesiologist beforehand to assure yourself that he comprehends the gravity of the situation.

Don't stop taking the Nardil for surgery that is imminent, as it takes about 10 days or so for the body to produce new MAO. For elective surgery that is able to be planned out somewhat in advance, yes, MAOIs are often discontinued. However, as with discontinuing any med, this should only be done in consultation with your doctor(s).

As Todd says there is no point in not continuing to take Nardil.

Make sure your anesthesiologist knows you are on a MAOI & you can't have anything that is a serotonin reuptake inhibitor or noradrenaline releaser.

Some studies suggest that anesthetics can be safely combined with MAOIs..A combination of ketorolac, midazolam, and propofol is often considered "safe"

...also bupivacaine has been used

MAOIs and surgery are discussed nearly every year of every doctor's training. The problem arises only because MAOIs are rare, so if you have emergent brought in unconscious surgery and the docs don't know your meds, they may not think of an MAOI as something you are on; but if they know about it beforehand--it's anesthesia 101. Also, the main culprit, epinephrine, I don't think is used in surgeries much anymore.

But I also wanted to say sorry about your accident. It sounds like a doozy. I hope surgery goes well and you have a speedy recovery.

This is probably the most important MAOI-related thread I've seen in the 2+ years I've been following Psychobabble. Most of the time, people talk about the diet restrictions, and sometimes contraindicated medications (OTC cold remedies). But I don't think we've had anything like this and it's something that's just as important if not more, than the hypertensive crisis concerns. I think that most of us MAOI patients, myself included, like to think, "oh this couldn't happen to me," i.e. we won't have to face the complications of anaesthesia from emergency surgery.
As a long-time believer in MAOIs, I wouldn't want this particular drawback of Parnate/Nardil to deter anyone from trying these meds, but we all have to think that emergencies can happen, get armed with all the facts,and be sure to impart all we know to the surgeon and anaesthesia personnel. I'd never be complacent about it or assume that they know.
Living here in Thailand, where MAOIs are not even available and some doctors are virtually ignorant about them, I've had to carry around detailed data on these drugs in the Thai language for emergency use.

Tepiaca, I hope the surgery went well. This is a perfect example of why it is so important to have something like a Medicalert bracelet or chain for events like this, particularly with meds like MAOI's, as an accident may occur where you can't communicate with the MDs' and the info can have a profound effect on how you are cared for.

Although I could only read the first 3 or 4 messages in that day , they helped me to think
more clearly.
Fortunately , my anestesiology made his MSC in Florida , and he really had knowledge about MAOIS
and surgeries .
Everything went fine . They used local anestesia
in my rigth leg ,and nothing bad happened.
I had 3 broken bones in that leg so they used pins (does that word exist? , you know , that things used with a hammer to fix tables etc etc..)
I can see right now how they are crossing my leg
through inside of it.
I have a terrible leg-ache so I cant sleep , it´s very painful.

Anyway thanks God , nothing bad happened in the quirurgic room .

Glad it turned out OK for you. I hate the sight of orthopedic surgery, those docs are brutal. :)

Thanks for updating us. I'm so glad you are okay, but sorry about the pain. I once observed orthopedic surgery. I had this idea of surgery being a delicate process. Oh no. It looked brutal. No wonder there is pain after.

Currently I´m on
- Ketorolaco (supradol) - for the pain .
I guess it is called ketorolac in US or something very similar

- Diclofenaco sodico (Voltaren Retard) -
for the inflamation

and finally

- Cefalexina (KEFLEX) -
cephalexine in US ( not sure about this)

yeah ! I remember how all my body shaked while they were inserting me all the nails into my leg
At that moment , my pain was not very severe.
The problem started after the anestesia lost its
effect. Oh my god ! that hurts !! I couldn´t sleep for two days
