I want to talk talk talk talk talk talk

Im a very quiet person ,Ive got something like
SP , I cant talk to the people , its very dificult to me . This is a nigthmare . Ive been living like this for many years . I want to have a normal life what do you suggest me to take ? now Im taking Zoloft (it helps but is not enough
and my sexual desire is gone) . And also one
medicine that contains 3 substances :Amitriptyline
Diazepam and perfhenazine.And also I take Amisulpride. Please help , any thoughts??????

Maybe instead of the Zoloft, you could switch to a tricyclic antidepressant like nortriptyline. You're already taking amitriptyline in combination with perphenazine and diazepam- nortriptyline is a metabolite of amitriptyline. I think the tricyclics are more effective for chronic and severe depression, and will probably have fewer sexual side effects than the Zoloft too.

Sounds like the Zoloft isn't working. My sister said it didn't work for her agoraphobia at all, so she switched to Paxil, and that really worked for her. Can't really give you more than that.

Just rememember that social phobia, like most phobias, requires you to also face the phobic situation. Maybe you'll have to to talk talk talk talk talk talk your way out of it. :D

As you have probably read, nardil is the most effective medication for social phobia. It also has the most side effects. After that it is trial and error, with meds such as klonopin, neurontin, SSRIs, parnate, effexor. Occassionally a TCA will help ... imipramine helps with GAD in certain studies.

I can't point to them, but I remember reading studies dismissing trycyllics in social anxiety...

It's important to remember that social anxiety is somewhere between the panic end of the spectrum and the calm end of the spectrum, so what sometimes works for GAD in some, like trycillics or Buspar, don't have a chance at social anxiety. It's complicated, and we don't know why at all. But my guess is GABA is MUCH more of an issue in social anxiety than GAD, and this isn't realized well, so we get serotonin crapalla.

Well, I never took Paxil, But I've tried Effexor for 5 months at 300mg. No effect, but made me not be able to ejaculate. So as a result, I couldn't concentrate (chemicals released after ejaculation for me assist my ADD-focusing) and got a GPA of 1.7 last semester. I really do want my doctor, who I'm switching out of now, to go away and never talk to me again.

I'm not risking it with Paxil, Celexa I've tried, and it didn't do anything, I took it for a year. Paxil is likely to give me sexual dysfunction, and not do anything for my anxiety, just like the others. Not worth it. I can't believe how anyone can be cured by Paxil for SP, it seems weird to me.

That is correct

But the diagnosis of Social Phobia and GAD often get mixed up with one another .... one may have GAD but get diagnosed with SP or vice versa. I have seen patients with dx of sp respond to TCAs.

As someone who's spent the past year and a half being on any med for the sole reason that it wasn't Klonopin, let me offer some advice:

long-acting drugs will never, ever help you. Nardil's interesting, but don't believe too much hype-- and even if it does work, you may get sick of not being able to do things like take stimulants whenever your college life demands a healthy supply (hypothetically, if you were in college) of stimulants. (Or as in Animal House, "weight loss drugs").

It'll never happen. Try Neurontin at hefty (400-1000mg/time) doses. It WILL make you less anxious, but your thoughts will be slow and you'll look drunk walking for a few days as your body adjusts. That can be avoided if you titirate in your sleep instead of during the day.

I just don't like Neuontin because it occassionally slows me down when I speak, so I say things in this embarassing monotonous voice, it's humiliating. But on the plus side, I feel great, comfortable around hot girls and talking to them, and sitting in class is the most chill thing ever.

Yes, Jumpy, that's a good point. I have been dx with GAD, but it has been manifesting itself lately as Social Phobia. I didn't have panic attacks, but I was afraid to see people who live in my building, so I would take garbage out in the middle of the night, use the stairs instead of the elevator, etc. My pdoc let me use the sample of Buspar he'd given me over the summer, when apparently his enthusiasm for it was a lot higher than it has been lately. Basically, I said I had the samples and they were gonna expire next month so I intended to use them if he didn't absolutely contraindicate it (I also take nortriptyline, an extremely low dose because I'm very sensitive to medications.) Well, within a day I was feeling better. Now, my dx is pretty complex: severe inattentive ADD, GAD, major depression; and only the nortriptyline had helped even a little.

It's my understanding though that TCA's are often more effective in severe melancholic depression. That is if you're sleepless and have poor appetite. I think it has a lot to do with body type too- I'm very skinny and can't gain weight to save my life. I was on a stimulant a number of years ago to treat my ADD, and lost 10 pounds and was ordered to go off it immediately. So, Tepiaca, the right medication can have to do with a lot more than just a generic 'dx.' There are factors like body type and and whether you can sleep at night to take into account. If I wasn't on a TCA, I would need a third medication just to get to sleep before 4 a.m.
