I' m upset , wanna find a good pdoc

Hi folks,

I am supposed to switch from Effexor to Parnate.
I've been tapering from a 300 mg. dose down to 75 mg. I' ve been on 75 mg. for three days by now and i'm feeling more apathetic and gloomy and kind of vulnerable. I went to the drug-store today and bought Parnate. I read the papers inside saying that the compound is a mixed one (tranylcypromine + a neuroleptic called Trifluoperazine Hydrochloride). The problem is that i've tried that kind of neuroleptic in the past and it made me very depressed. I told my pdoc about that and he told me that i wasn't supposed to read that paper, since i was supposed to follow his prescriptions. "Let me do my job and you do yours which is being a patient !!!" - he yelled. Then he added: "go ahead and take it when you're supposed to take which will be the next week" "But what happens if i get sick?" i said. "Don't worry about that. People have been taking that med for a long time and never had any problem. If you're going to feel sick we'll see what we can do. I'm the doctor, not you" - he shouted.
When i hang up the phone i was in tears. I think i''ll go back to effexor, i'm not going to take that neuroleptic cause i'm sure i'll get sick again and i don't want that. Why is it so hard to find a decent pdoc? Is it my fault?

Is it your fault? Depression talking...

No, it's not your fault! Your pdoc sounds somewhat like a control freak who does not respect you. FInd someone you can discuss with, someone who will let you manage your health with his/her professional assistance. Patients are clients; they tend to forget that sometimes. If a hairdresser does a bad job with your hair you go find another one. Same thing with doctors.

Perhaps you should still try this med, perhaps it can help you and not necessarily make you sick. That does not give the pdoc the right to yell at you and to talk to you the way he did.

Again, this is not your fault. Blaming yourself for things that you can't possibly control is part of the depression. Try stopping these thoughts before they hurt you, as soon as you catch yourself thinking them. And find another doctor.

Hello, Anna Laura,

I agree with Sid. A pdoc who would shout at a patient who is just concerned and afraid? -- absolutely unforgiveable, in my opinion.

(Could be, as Sid suggests, that this pdoc is a control freak. Sounds to me like a character or mood disorder, perhaps both!)

Also, this doctor is extremely poorly informed about patient's rights. In fact, he is quite possibly committing MALPRACTICE by telling you not to read the inserts. Why does he suppose the inserts are there? Because they are required by law! because the patient has a right to know of the risks. because the doctor can be sued if the patient has not given an informed consent.

Also, parnate is an MAOI, is it not? (I am not sure, but I seem to recall that it is.) And if so, the doctor must give you very, very precise instructions on foods that you must avoid. MAOIs are very useful drugs, but can be dangerous and even fatal when combined with certain foods.

I hope that you will be able to find a pdoc who, at a minimum, has good manners, Anna. That is the least that any of us are entitled to.

I think any doctor who yells at his patients isn't fit to be a doctor. If he can't handle himself how can he advise others on what to do?

I know you live in Italy. What's the doctors there like? Is it easy to find another caring doctor? Is there any therapy groups around you that you could get in touch with to find what they'd recommend as a good doctor. There must be something that gives lists of available doctors. You can even do a search on your internet for doctors in your area.

A lot of what the doctor said was valid. His explanations to you & his method of explaining definitely ARE NOT though.
...And I'm always leery of any doctor who doesn't like to see his patients learn about their meds. I call it a "God syndrome". None of what you asked him was out of place but showed you're thinking ahead, that's all. What a jerk he must be!

The pdoc scene here in Italy it's quite discouraging. I believe there are many valuable and professional pdoc here, the only problem is that our legislation doesn't protect us from pdoc malpractice and abuse AT ALL. That really sucks, believe me.God only knows all the abuses I've been through. Doctors here are worshipped as Gods, and the patients are supposed tobe submissive and humble, following their instructions without complaining. The implications for such cultural and legal customs is that pdocs feel somehow "protected" thus feeling legitimated to pursue their whims. The pdoc i was talking about didn't even know about dopaminergic augmentation of ADs. When i mentioned that, he told me i was a fool. Funny thing is that i suggested MAOIS to him since he had told me i've had tried all the ADs available and that there were no other meds i could try. "Right, i didn't think about that "- was his reply."Parmodaline is the only MAOI available in the market though"- he added. I told him there was Selegiline which was a type B MAOI which he didn't know about. "I'll make some research on that and i'll let you know " he replied.
The second time i called him he told me i was stupid and ignorant since Selegiline was a drug for Parkinson desease. I made a research through the italian drugs data base, which showed Selegiline classification being controversial since it was a MAOI ( thus being an antidepressant) even though it was being prescribed for Parkinson desease.
Last spring I went to IDEA association which is a sort of national alleance of depressed patients only to find out that i already knew the pdoc they suggested me. This jerk misdiagnosed my depression, labelled it as a anxiety disorder and prescribed me a strong neuroleptic resembling thorazine which turned me in to a vegetable. I feel like ther's nowhere to turn. I'm considering about going abroad since i live near the French border (three hours drive away). I don't know what else to do.

Anna, that sounds like a horrible circumstance you're in. It wasn't that long ago that doctors in North America had the same God-syndrome as yours has. Now patients are acting as consumers, hiring or firing their doctor if they're not satisfied & doctors now have to listen to patients more or they lose business. Specialists can still have that syndrome as there's not as many so that includes psychiatrists.

Here in Canada, most general physicians prescribe a psychotropic drugs & only refer their patients to psychiatrist if they don't respond to medication & the meds need to be carefully balanced. How is it in Italy? Can a regular doctor prescribe the needed medication? I've found these doctors are a little more humble & will listen to their patients & read articles brought to their attention more than psychiatrists. Maybe your regular doctor, if he doesn't want to prescribe, can recommend a fellow physician who would.

Anna, it bothers me that you've got such a lousy doctor. I can't promise I can find anything but do you mind me asking what major centre you live near or in, in Italy? It makes it easier for me to find info.

Go ahead i don't mind. Thank you so much for taking care of my situation. I'll so some reaserch myself: i've been depressed for eleven years: i think it's about time for me to start feeling better.

You were asking about a couple of years ago about the possibility to get MAOIs in Italy. I'm in the same situation since I live in Italy and I'm taking Parmodalin but I managed to discover that it's possible to purchase Nardil in the Vatican Pharmacy. I don't know if you're stil following Psychobabble but if so I hope this helps. You were also looking for a good and kind psychiatrist and I would have a suggestion for you if you have not yet managed to find it. Best wishes, wherever you are
