topirimate in Bipolar Disorder

Posted by Chris A. on January 2, 1999, at 4:20:12

Hi!

I'm new here. Does anyone know what the latest
research/clincal experience with topirimate is showing?
Is it proving to be helpful for severe bipolar depression
in which any use of ADs and even lamotrigene induced rapid
cycling/mixed states? We have tried everything else
except nimopidine and rTMS (the list is comprehensive).
Has anything been published in the past year?

Thanks,
Chris

Some good studies on Topirimate have been published this year, using it alone and as an add-on to other mood stabilizers that are incompletely effective. I haven't seen where it is effective for depression alone, but I have seen it stop mania dead in its tracks (both these patients were on it without another mood stabilizer). The researchers say that it causes cognitive dulling but in my patients, I've warned them ahead of time and all have reported back that it has not happened to them (a sample size of only 9 patients, though). Good luck.

Thanks for sharing your experience with topiramate,
Toby.

My "review and strategy" meeting with my psychiatrist
is tomorrow and the more informed we are, the better.

Best,

Chris A.
