Hi-

Sending a handwritten letter, should be as effective, however, they can always 'claim' they never received it. I sent everything certified.

Perhaps you could call to verify they received the letter? Ask if there are any problems, etc... if they say they haven't received it, send it certified... much harder to dispute.

I can't think of a particular form... however, I'm not an expert(not legally..hehe, just dealt with this for what seems an eternity) You can search on the web for a template, or if you'd like to exchange email addresses, I can send you the one I have. Anything I can do to help out.
