And something you counted on (your husband working) you can't count on any more. Most important, it seems like geriatrics is not your field. I think it is very very important to like your job. Perhaps you can see this job as temporary until you can get back to a job that fufills you, like pediatrics. I think seeing real stresses in life as necessary but not permanent allows us to pass through them easier.
