What was she trying to figure out?

I was finally able to explain to my T that when she asks me what I'm thinking, and I say "I don't know," it usually means that I want to tell her, but I don't know how to express my thoughts. She said it's hard to believe my mind is just a complete blank. I told her I'm not sure if it's a complete blank, or I just have too many thoughts swirling around.

She asked me if I sort of comment to myself when I'm reading a magazine... even just something simple like "oh I like those shoes," or if I just read the words.

What was she trying to figure out when she asked me that? Is that a symptom of something?
