OTC painkillers are often anti-inflammatory in nature and as the pain is emotional ( from your head not a physical illness )it cannot kill that pain. Now say you had a psychosomatic pain where the emotion causes the illness then it would work....because your mind would have made your body sick. I think from what you explain if I understand if they took an xray they would see no infammation but if you worried yourself into a ulcer that could be treated with meds I cant think of a better way to put this.
