I really can sympathise with your problem. I've had it for many years too and at its worst it truly is soul destroying.
Tell me, do you drink alcohol? How do you feel after the occasional session of drink? Does the drink remove the anxiety? How long does it stay gone after an evening of drinking?
Also supps, have you found any that help in the long term or are they patchy in performance?
The reason I ask, is I often end up taking lots of supps to get me through lengthy tough spells but end up finding the supps are prolonging or worseoning the bad spell and that quiting the supps temporarily leaves me feeling great (which seems odd).
Can't get my head around it fully BUT the one thing I have got my head around is that however bad we may feel, its the fear of the anxiety that acually keeps us feeling at our worst. Not quite sure how to consistently break the cycle, but to have a good drink isn't a bad OCCASIONAL strategy to break the chain once in a while provided one has the strength of mind to know that the real long term answers are not to be found at the bottom of the bottle.
