Porkpiehat, Have you experienced this type of switch in mood before? Is there any bipolarity in your family? If there is, it may change treatment strategies. If not, then desipramine or nortriptyline might be worth trying. You can simply add one of these to Effexor so that you won't go without some coverage. If ADHD/ADD is involved, I would go with desipramine.

Some people, including myself, find Focalin (dexmethylphenidate) smoother and more consistent than Ritalin (methylphenidate). Whatever it takes. Vyvanse (lisdexamfetamine) was not at all helpful to me.

What illness and symptoms are you trying to treat? In some situations, Provigil or Nuvigil help with anergic and cognitive symptoms. If word-finding impairments are particularly problematic, Namenda (memantine) often helps.
